一次性输尿管软镜联合负压吸引鞘与超细经皮肾镜处理肾下盏结石的对比研究

Comparison between disposable flexible ureteroscope combined with negative pressure suction sheath and ultrafine percutaneous nephroscope in the treatment of lower calyceal calculi

摘要 目的比较一次性输尿管软镜联合负压吸引鞘与超细经皮肾镜处理肾下盏结石的临床效果。方法回顾性选取2021年9月至2023年9月衢州市人民医院收治的121例肾下盏结石患者,其中采用一次性输尿管软镜联合负压吸引鞘诊治74例(联合组),采用超细经皮肾镜诊治47例(肾镜组),比较两组手术情况和预后。结果两组患者术后第1天血红蛋白下降量、术后第1天C反应蛋白、降钙素原、术后发热例数比较,差异均无统计学意义(均P>0.05);联合组手术时间、住院时间均显著短于肾镜组,差异均有统计学意义(均P<0.05),术后1个月清石率肾镜组高于联合组,差异有统计学意义(P<0.05)。结论超细经皮肾镜和一次性输尿管软镜联合负压吸引鞘都是处理肾下盏结石安全、可靠的手术选择,一次性输尿管软镜联合负压吸引鞘在手术时间及住院时间上存在优势,超细经皮肾镜在术后清石率上存在优势。 Objective To compare the clinical effects of disposable flexible ureteroscope combined with negative pressure suction sheath and ultrafine percutaneous nephroscope in the treatment of lower calyceal calculi.Methods A total of 121 patients with lower calyceal calculi admitted to Quzhou People's Hospital from September 2021 to September 2023 were retrospectively selected.They were divided into a disposable flexible ureteroscopy combined with negative pressure suction sheath group(Group A,74 cases)and an ultrafine percutaneous nephroscopy group(Group B,47 cases).The operation situation and prognosis were compared between the two groups.Results There were no significant differences between the two groups in the amount of hemoglobin decrease,C-reactive protein,procalcitonin and the number of fever cases on the first day after operation(all P>0.05);The operation time and hospitalization time of Group A were shorter than those of Group B(both P<0.05),and the stone clearing rate of Group B was higher than that of Group A one month after surgery,with statistically significant differences(P<0.05).Conclusion Ultrafine percutaneous nephroscope and disposable flexible ureteroscope combined with negative pressure suction sheath are both safe and reliable surgical options for treating lower calyx calculi.Disposable flexible ureteroscope combined with negative pressure suction sheath has advantages in surgical time and hospital stay,while ultrafine nephroscope has advantages in postoperative stone clearance rate.
