摘要
疾病的治疗过程离不开医患双方的共同决策,其中尤以恶性肿瘤的决策最为复杂,归咎于其长时间的疗程,居高不下的治疗费用以及无法明确的预后等,这也促使其成为医学工作者研究的热点之一。进入21世纪以来,医学的进步为恶性肿瘤的治疗提供了全新的方法,面对此新老交替的局面,医患共同决策也将面临新的机遇和挑战。如何在发展变化中更为慎重地深入考量,理性决策,保障乃至扩大患者获益,值得探讨。以局部晚期口腔鳞癌新辅助治疗为例,就医患双方探讨在治疗过程中以客观实际为基础开展医患共同决策,以期为临床工作提供一定的参考。
The treatment of diseases inevitably involves shared decision-making between physicians and patients,a process that becomes particularly complex in the case of malignant tumors.Factors such as prolonged treatment duration,high medical costs,and uncertain prognoses contribute to this complexity,making shared decision-making a prominent topic of research in clinical medicine.Since the beginning of the 21st century,advances in medical science have introduced new approaches to cancer treatment.In this evolving landscape,shared decision-making faces both new opportunities and challenges.How to navigate this changing context with greater prudence,engage in rational deliberation,and ensure—or even enhance—patient benefit warrants careful consideration.Taking the example of neoadjuvant therapy for locally advanced oral squamous cell carcinoma,this paper explores how physicians and patients can engage in shared decision-making based on objective and practical considerations,aiming to offer insights for clinical practice.
