摘要
目的 探讨经会阴宫颈剪切波弹性成像联合宫颈长度及宫颈角对孕中期孕妇早产风险的预测价值。方法 选取115例建档孕妇,随访至分娩,根据分娩时间分为早产组34例和足月组81例,收集其临床病史、二维超声检查、宫颈弹性成像资料,运用Logistic回归分析筛选出早产高危因素,绘制受试者工作特征(ROC)曲线,分析宫颈长度、宫颈角、宫颈弹性模量及三者联合对早产的预测价值。结果 早产组宫颈长度小于足月组[(26.53±3.40)cm vs(31.59±4.24)cm];早产组宫颈角大于足月组[(104.17°±7.10°)vs(97.32°±6.70°)];早产组宫颈内口弹性模量值小于足月组[(6.40±1.10)vs(7.69±1.03)],差异均有统计学意义(P均<0.05);早产组宫颈外口弹性模量值[(6.42±1.19)kPa]小于足月组[(6.99±1.14)kPa],差异无统计学意义(P>0.05);宫颈长度、宫颈角、宫颈内口弹性模量值及三者联合对预测早产的ROC曲线下面积分别为0.773、0.780、0.809、0.939。结论 经会阴宫颈弹性模量联合宫颈长度及宫颈角可提高对孕妇早产风险的预测效能。
Objective To investigate the value of transperineal cervical elastography combined with cervical length diameter and uterine cervical angle in mid-pregnancy for predicting the risk of preterm labor in pregnant women.Methods A cohort of pregnant women were followed up until delivery.Based on gestational outcomes,34 women with preterm birth and 81 women with full-term delivery were included.Clinical histories,2D ultrasound imaging,and cervical elastography data were collected.Logistic regression was used to identify risk factors for preterm birth,and ROC curves were generated to evaluate the effect of CL,UCA,cervical elasticity and their combined effects in predicting the occurrence of preterm labor.Results The preterm group exhibited a significantly shorter CL than the full-term group[(26.53±3.40)cm vs(31.59±4.24)cm]and a larger UCA[(104.17°±7.10°)vs(97.32°±6.70°)],while the cervical internal os elasticity was significantly lower in the preterm group[(6.40±1.10)vs(7.69±1.03)](P<0.05).No significant difference was found in external os elasticity between the groups[(6.42±1.19)vs(6.99±1.14)](P>0.05).The area under the ROC curve for CL,UCA,internal os elasticity,and their combined application on the occurrence of preterm labor were 0.773,0.780,0.809,and 0.939,respectively.Conclusion The combination of transperineal cervical elastic modulus,cervical length,and cervical angle significantly improves the predictive efficiency for assessing the risk of preterm birth in pregnant women,demonstrating its potential as a valuable tool in clinical practice.
