MRI对胎儿侧脑室增宽合并其他颅内畸形的诊断价值

摘要 目的 探讨MRI对胎儿侧脑室增宽合并其他颅内畸形的诊断价值。方法 选取185例经超声检查及MRI诊断为侧脑室增宽的胎儿临床及影像学数据,依据首次检查的胎儿侧脑室宽度分为轻度(10.0~11.9 mm)、中度(12.0~14.9 mm)及重度(≥15.0 mm)组,分析不同组别胎儿合并其他颅内畸形的发生率,并对照超声检查结果分析MRI对各组胎儿合并其他颅内畸形的诊断价值。结果 185例胎儿中,侧脑室轻度增宽122例(轻度组),中度增宽42例(中度组),重度增宽21例(重度组)。三组胎儿中,超声检查分别检出伴发其他颅内畸形5、3及4例,MRI检出10、11及12例。三组胎儿合并其他颅内畸形的发生率、超声检查以及MRI对于中、重度组胎儿合并其他颅内畸形的检出率比较差异有统计学意义(P均<0.05)。结论 胎儿颅内畸形的发生率随侧脑室宽度的增加而增加,尤其是对于中、重度侧脑室增宽的胎儿应及时行MRI检查,有助于明确是否合并其他颅内畸形。 Objective To evaluate the diagnostic value of fetal magnetic resonance imaging(MRI)for fetal ventriculomegaly combined with additional intracranial malformations.Methods The clinical and imaging data of 185 fetuses diagnosed as ventriculomegaly by ultrasound and MRI were collected retrospectively.According to the first examination result,they were divided into mild(10.0-11.9 mm),moderate(12.0-14.9 mm),and severe group(≥15.0 mm).The incidence of additional intracranial malformations in different groups of fetuses were compared and analyzed,and the diagnostic value of MRI in each group with ultrasound was compared.Results Among 185 fetuses,there were 122 cases of mild group,42 cases of moderate group,and 21 cases of severe group.Among the three groups of fetuses,ultrasound detected 5,3,and 4 cases of additional intracranial malformations,while,MRI detected 10,11,and 12 cases,correspondingly.There were statistical differences in the incidence of additional intracranial malformations among the three groups,as well as the detection rates of intracranial malformations in fetuses with moderate and severe ventriculomegaly using ultrasound and MRI(P values<0.05,respectively).Conclusion The incidence of fetal intracranial malformations increases with the increase of lateral ventricular width,especially for fetuses with moderate to severe ventriculomegaly.MRI examination should be performed timely to help clarify whether additional intracranial malformations are present.
