摘要
目的 探讨温敏性水凝胶用于经导管动脉化疗栓塞术(TACE)治疗原发性肝癌的临床效果及安全性。方法 选取经临床确诊为不可切除的25例中晚期原发性肝癌患者,首次接受TACE中使用温敏性水凝胶。按照mRECIST标准评价中远期预后情况,并记录术后不良反应。结果 患者均顺利完成肝癌TACE,术后不良反应主要有胃肠道反应、疼痛、发热等,反应程度较轻,患者无肝破裂、肝脓肿等严重并发症和治疗相关死亡。25例患者治疗后1、3、6个月客观缓解率分别为80.0%、63.6%、38.9%,疾病控制率分别为92.0%、86.4%、72.3%,中位生存时间为13个月,中位无进展生存期为7个月。结论 温敏性水凝胶用于经导管肝动脉栓塞治疗原发性肝癌安全可行,临床疗效显著。
Objective To evaluate the effectiveness and safety of thermosensitive hydrogel for transcatheter arterial chemoembolization(TACE)in the treatment of primary liver cancer.Methods Twenty-five patients with clinically confirmed unresectable advanced hepatocellular carcinoma who were receiving their first TACE treatment were selected.A thermosensitive hydrogel was used during the procedure.The medium-and long-term prognosis was assessed according to the mRECIST criteria,and postoperative adverse reactions were recorded.Results All patients successfully completed the TACE procedure for liver cancer.Postoperative adverse reactions mainly included gastrointestinal reactions,pain,and fever,which were mild in severity.No patients experienced serious complications such as liver rupture or liver abscess,nor were there any treatment-related deaths.The objective response rates(ORR)at 1,3,and 6 months after treatment were 80.0%,63.6%,and 38.9%,respectively,while the disease control rates(DCR)were 92.0%,86.4%,and 72.3%,respectively.The median overall survival(mOS)was 13 months,and the median progression-free survival(mPFS)was 7 months.Conclusion Thermosensitive hydrogel for transcatheter arterial chemoembolization in the treatment of primary liver cancer is safe and feasible,with significant clinical efficacy.
