摘要
目的 探讨磁共振成像(MRI)特征、磁共振弥散加权成像(DWI)定量分析参数鉴别诊断腮腺肿瘤良恶性的价值。方法 选取腮腺肿瘤患者60例,分析腮腺肿瘤定性诊断结果,比较不同类型腮腺肿瘤MRI影像学特征、DWI定量分析参数。通过MRI、DWI、MRI联合DWI检查鉴别腮腺肿瘤良恶性,与病理诊断结果比较。以受试者工作特征(ROC)曲线为依据,评估MRI、DWI单独及联合检测对腮腺肿瘤良恶性的诊断价值。结果 60例腮腺肿瘤患者,共计89个肿块,恶性肿块26个,其中黏液表皮样癌15个(57.69%),腺泡细胞癌7个(26.92%),其他4个(15.38%);良性63个肿块,其中腺淋巴瘤41个(65.08%)、多形性腺瘤16个(25.40%)、其他6个(9.52%)。恶性腮腺肿瘤边界清晰、形态规则、信号均匀、无周围组织侵犯、无分叶比例低于良性,差异有统计学意义(P<0.05)。b=600、800、1 000 s/mm^(2)时,恶性腮腺肿瘤表观弥散系数(ADC)值低于良性腮腺肿瘤,差异有统计学意义(P<0.05)。经病理检查证实,89个腮腺肿瘤肿块,恶性肿块26个,MRI确诊16个,DWI确诊18个,MRI联合DWI确诊24个。ROC曲线分析显示MRI联合DWI检测的曲线下面积(AUC)为0.898,灵敏度为92.31%,特异度为87.30%。结论 不同类型腮腺肿瘤MRI特征、DWI定量分析参数存在不同,MRI联合DWI检查鉴别腮腺肿瘤患者良恶性的价值较高。
Objective To explore investigate the magnetic resonance imaging(MRI)features and quantitative parameters of magnetic resonance diffusion weighted imaging(DWI)in patients with benign and malignant parotid tumors.Methods 60 patients with parotid gland tumors were admitted.Clinical data of the study subjects were retrospectively collected.The qualitative diagnostic results of parotid tumors were analyzed,the MRI image characteristics of different types of parotid tumors and the quantitative analysis parameters of DWI were analyzed,and the differential diagnosis of benign and malignant parotid tumors by MRI,DWI,and MRI combined with DWI was compared with the pathological diagnosis,and the receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of MRI and DWI alone and combined detection in benign and malignant parotid tumors.Results There were 60 patients with parotid gland tumors,including 89 masses.There were 26 were malignant masses,including 15 of mucoepidermoid carcinoma(57.69%),7(26.92%)of acinar cell carcinoma,and 4(15.38%)of other types.There were 63 benign masses,including 41 of adenomas(65.08%),16 of pleomorphic adenomas(25.40%)and 6 of other tumors(9.52%).There were 63 benign masses,including 41 of adenomas(65.08%),16 of pleomorphic adenomas(25.40%)and 6 of other masses(9.52%).The malignant boundary of parotid gland tumor was clear,the shape was regular,the signal was uniform,there was no surrounding tissue invasion,and the proportion of no lobed was lower than that of benign parotid gland tumor(P<0.05).When b=600,800,1000 s/mm2,the apparent diffusion coefficient(ADC)of parotid tumor was lower than that of benign parotid tumor(P<0.05).Among the 89 parotid tumor masses confirmed by pathological examination,26 were malignant,16 were confirmed by MRI,18 by DWI,and 24 by MRI combined with DWI.Malignant parotid tumors were included as positive and benign parotid tumors as negative.ROC curve was drawn,the area under the curve(AUC)of MRI combined with DWI was 0.898,the sensitivity was 92.31%,and the specificity was 87.30%,which had high value in differentiating benign and malignant parotid tumors.Conclusion MRI features and DWI quantitative analysis parameters were different in different types of parotid tumors.MRI combined with DWI examination was of high value in differentiating benign and malignant parotid tumors,which could lay a foundation for its diagnosis and treatment.
