摘要
目的探讨汽车制造业工人长期累积噪声暴露量(cumulative noise exposure,CNE)与血清三酰甘油(TG)水平之间的关联性,为预防TG异常提供科学依据。方法采用方便抽样法,选取某大型汽车制造企业1207名工人为研究对象。通过健康检查、问卷调查、现场噪声检测收集相关信息。采用Pearsonχ^(2)检验对高CNE组与对照组TG异常率进行比较;以TG是否异常为自变量,建立CNE对TG异常影响的logistic回归模型;利用倾向得分匹配(propensity score matching,PSM)法控制混杂因素后,以χ^(2)检验分析CNE与TG异常的关联性。结果单因素模型显示,高CNE的OR(95%CI)值为2.01(1.41,2.87),调整年龄、性别、超重或肥胖因素后,高CNE的OR(95%CI)值为1.79(1.24,2.60);采用PSM方法控制混杂因素后,高CNE的OR(95%CI)值为1.67(1.01,2.76),P=0.044。结论CNE与TG异常具有正向关联性,企业应控制工作场所噪声水平,减少工人接噪时间,以降低TG异常发生风险。
Objective To explore the relationship between cumulative noise exposure(CNE)and serum triglyceride(TG)levels of workers in the automobile manufacturing industry,and provide a scientific basis for the prevention of abnormal triglyceride caused by occupational noise exposure.Methods One thousand two hundred and seven workers in a large automobile factory were selected by convenient sampling method.Information was collected through health checks,questionnaires,and field noise detection.Statistical methods:Pearson Chi-square test was used to compare the TG anomalies rate between the high CNE group and the control group.Then a logistic regression model of the influence of CNE on TG anomalies was established with TG anomalies as the independent variable.After controlling confounding factors by propensity score matching(PSM)method,Chi-square test was used to examine the correlation between CNE and TG anomalies.Results The univariate model showed that the OR(95%CI)value of high CNE was 2.01(1.41,2.87),and 1.79(1.24,2.60)after adjusting for age,sex,overweight or obesity.After controlling for confounding factors by PSM,the OR(95%CI)value of high CNE was 1.67(1.01,2.76),P=0.044.Conclusion There is a positive relationship between cumulative noise exposure and triglyceride abnormality.Enterprises should control workplace noise level and reduce workers'noise exposure time,so as to reduce the risk of triglyceride abnormality.
