不同型号血液灌流器对急性中毒患者血小板的影响

Effect of different models of hemoperfusion apparatus on platelets in patients with acute poisoning

摘要 将2021年12月至2024年6月在襄阳市中心医院进行血液灌流(HP)的175例急性中毒患者依据使用灌流器的型号分为HA330灌流器组(对照组)和HA380灌流器组(观察组),比较两组灌流前后PLT的差异。结果显示,两组单次灌流前后PLT计数差异均有统计学意义;观察组与对照组比较,单次灌流后PLT计数、PLT计数下降值及下降比例、PLT低于正常值的例数,差异均有统计学意义(P<0.05)。Logistic回归分析显示,不同型号灌流器、灌流前PLT计数是导致急性中毒灌流后PLT下降的独立危险因素(P<0.05)。与HA330灌流器相比,HA380灌流器对PLT影响更小,可能减少患者出血风险和使用血制品的机会,利于顺利完成血液灌流。 A total of 175 patients with acute poisoning who underwent hemoperfusion in Xiangyang Central Hospital from December 2021 to June 2024 were collected.According to the perfusion type used,the patients were divided into HA330 perfusion group(control group)and HA380 perfusion group(observation group).The differences of platelets(PLT)before and after single perfusion between the two groups were compared.The results showed that the platelet counts before and after single perfusion were statistically different between control group and observation group.Compared with the control group,there were statistical differences in platelet count,platelet count decline value,platelet count decline proportion and the number of cases with platelet count lower than the normal range after single perfusion(P<0.05).Logistic regression analysis showed that different types of perfusion device and platelet count before perfusion were independent risk factors for platelet count below the normal range after acute poisoning perfusion(P<0.05).The use of the HA380 or HA330 perfusion device will lead to a decrease in platelet count,but the selection of the HA380 perfusion device will have less impact on platelets,which may reduce the risk of bleeding and the use of blood products,and also facilitate the planned hemoperfusion.
