摘要
小儿鼻鼽以鼻塞、流清涕、鼻痒、打喷嚏为主要表现,病因为卫闭阳郁、清阳不升、湿热蒸腾、瘀血阻络。水液代谢失常为流清涕的重要病因,内外风扰是引起鼻痒、打喷嚏的主要因素。据此制定祛邪开塞,以通为用治鼻塞;通调水道,引液归经止流涕;内外兼顾,祛风散邪,补虚泻实安鼻痒等治则治法。两症或数症并见时,须分清主次,急攻主症,缓解次症,急性期泻实,缓解期补虚,慎用攻伐,中病即止。附案例1则,以资验证。
Pediatric allergic rhinitis is primarily characterized by nasal congestion,clear nasal discharge,nasal itching,and sneezing.The pathogenesis involves defensive qi obstruction and yang stagnation,failure of clear yang to ascend,damp-heat steaming,and blood stasis blocking the collaterals.Dysregulation of fluid metabolism is a key factor in clear nasal discharge,while internal and external wind disturbance is the main cause of nasal itching and sneezing.Based on this,the treatment principles and methods are established:expelling pathogens and unblocking obstruction to relieve nasal congestion;regulating water passage and redirecting fluids to stop nasal discharge;addressing both internal and external factors,dispelling wind and pathogens,reinforcing deficiency and reducing excess to alleviate nasal itching.When multiple symptoms coexist,priority should be given to the dominant symptom for urgent treatment while managing secondary symptoms,with excess patterns treated aggressively in acute phases and deficiency patterns reinforced during remission.Overly aggressive therapies should be avoided,and treatment should cease once efficacy is achieved.A case example is provided for validation.
