摘要
目的探讨糖化血红蛋白(glycated hemoglobin A1c,HbA1c)联合糖耐量检测在妊娠期糖尿病(gestational diabetes mellitus,GDM)诊断中的应用价值。方法采用回顾性分析方法,收集2024年1—12月期间在汕头市妇幼保健院就诊的117例GDM孕妇资料作为研究组,并同期收集了43例非GDM孕妇资料作为对照组。对两组均进行HbA1c以及糖耐量检测,并对比分析两组间各指标的水平差异,同时评估上述指标单独及联合检测的诊断效能。结果研究组HbA1c、空腹血糖、服糖后1 h及2 h血糖值均高于对照组,差异均有统计学意义(P均<0.05)。联合检测的诊断灵敏度98.29%、特异度100.00%、准确度98.75%、阳性预测值100.00%、阴性预测值95.56%高于单一检测,差异均有统计学意义(χ^(2)=5.856,4.921,6.578,4.854,4.411;P均<0.05)。结论HbA1c联合糖耐量检测在GDM的诊断中具有较高的应用价值,能够显著提高诊断的灵敏度、特异度和准确度,有助于早期发现和管理GDM。
Objective To explore the application value of glycated hemoglobin A1c(HbA1c)combined with glucose tolerance test in the diagnosis of gestational diabetes mellitus(GDM).Methods A retrospective analysis method was used to collect the data of 117 GDM pregnant women who were treated in Shantou Maternal and Child Health Hospital from January to December 2024 as the study group,and the data of 43 non-GDM pregnant women were collected as the control group.Both groups were tested for HbA1c and glucose tolerance,and the differences in the levels of each index between the two groups were compared and analyzed.At the same time,the diagnostic efficacy of the above indicators alone and in combination was evaluated.Results The levels of HbA1c,fasting blood glucose and blood glucose at 1 h and 2 h after taking glucose in the study group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined detection were 98.29%,100.00%,98.75%,100.00%and 95.56%,respectively,which were higher than those of single detection,and the differences were statistically significant(χ^(2)=5.856,4.921,6.578,4.854,4.411;all P<0.05).Conclusion HbA1c combined with glucose tolerance test has high application value in the diagnosis of GDM,which can significantly improve the sensitivity,specificity and accuracy of diagnosis,and help early detection and management of GDM.
