摘要
目的探讨基于床边超声引导技术的精准穿刺置管引流术(ultrasound-guided precise puncture and catheterization drainage,USPCD)对急诊创伤性血气胸患者的疗效与安全性。方法方便选取2021年12月—2023年6月于松滋市人民医院急诊就诊的83例创伤性血气胸患者为研究对象,根据不同引流方法分组,对照组(n=40)接受传统胸腔闭式引流术治疗,研究组(n=43)接受USPCD治疗。对比两组置管相关指标、不适症状消失时间、一次性置管成功率及并发症发生情况。结果研究组置管相关指标优于对照组,差异均有统计学意义(P均<0.05)。术后,研究组不适症状消失时间短于对照组,差异均有统计学意义(P均<0.05)。研究组的一次性置管成功率为100.00%(43/43),高于对照组的85.00%(34/40),差异有统计学意义(χ^(2)=4.896,P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论USPCD治疗急诊创伤性血气胸患者,不仅改善了置管相关指标,提高了置管成功率,缩短了患者不适症状的消失时间,还降低了并发症发生率。
Objective To investigate the efficacy and safety of bedside ultrasound-guided precise puncture and catheterization drainage(USPCD)in emergency patients with traumatic hemopneumothorax.Methods A total of eighty-three patients with traumatic hemopneumothorax who were treated in the Emergency Department of Songzi People's Hospital from December 2021 to June 2023 were conveniently selected as the research objects.According to different drainage methods,the control group(n=40)received traditional closed thoracic drainage treatment,and the study group(n=43)received USPCD treatment.The related indexes of catheterization,the disappearance time of discomfort symptoms,the success rate of one-time catheterization and the occurrence of complications were compared between the two groups.Results The related indexes of catheterization in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).After operation,the disappearance time of discomfort symptoms in the study group was shorter than that in the control group,and the differences were statistically significant(all P<0.05).The success rate of one-time catheterization in the study group was 100.00%(43/43),which was higher than 85.00%(34/40)in the control group,and the difference was statistically significant(χ^(2)=4.896,P<0.05).The incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion USPCD is effective in the treatment of patients with traumatic hemopneumothorax in emergency department.It not only improves the relevant indicators of catheterization,improves the success rate of catheterization,shortens the disappearance time of patients'discomfort symptoms,but also reduces the incidence of complications.
