无创正压通气治疗慢性阻塞性肺疾病的效果

摘要 目的:评估无创正压通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)的临床效果。方法:选取2023年1月—2025年2月于复旦大学附属中山医院青浦分院住院治疗的98例COPD患者,通过随机数字表法分为对照组(n=49)和观察组(n=49)。在常规药物治疗基础上,对照组予以鼻导管吸氧,观察组予以NIPPV。比较两组治疗前后血气分析指标(动脉血氧饱和度、动脉血二氧化碳分压、动脉血氧分压)、肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC、最大呼气中期流量]、住院时间及不良事件发生率。结果:治疗后,相较于对照组,观察组动脉血氧饱和度及动脉血氧分压更高(P<0.001),动脉血二氧化碳分更低(P<0.001),FEV1、FVC、FEV1/FVC以及最大呼气中期流量更高(P<0.001);观察组住院时间较对照组更短(P<0.001);两组不良事件发生率比较,无统计学差异(P>0.05)。结论:NIPPV能有效改善COPD患者血气分析和肺功能指标,缩短住院时间,且未增加不良事件发生风险。 Objective:To evaluate the clinical effect of non-invasive positive pressure ventilation(NIPPV)in treatment of chronic obstructive pulmonary disease(COPD).Methods:This study enrolled 98 patients hospitalized in Qingpu Branch,Zhongshan Hospi‐tal,Fudan University from January 2023 to February 2025 were selected.Participants were allocated to a control group(n=49)and an observation group(n=49)by using random number table method.On the basis of conventional drug therapy,the control group was treated with oxygen inhalation via nasal catheter and the observation group was treated with NIPPV.The blood gas analysis in‐dexes(arterial oxygen saturation,arterial partial pressure of carbon dioxide,arterial partial pressure of oxygen)and pulmonary func‐tion indexes[forced expiratory volume in the first second(FEV₁),forced vital capacity(FVC),FEV₁/FVC ratio,maximum mid-expiratory flow]before and after treatment,as well as the length of hospital stay and incidence of adverse events were compared be‐tween the two groups.Results:Following treatment,compared with the control group,the observation group had higher arterial oxy‐gen saturation and arterial oxygen partial pressure(P<0.001),lower arterial carbon dioxide partial pressure(P<0.001),and higher FEV1,FVC,FEV1/FVC,and peak expiratory flow rate(P<0.001).The observation group had a shorter hospital stay than the control group(P<0.001).There was no statistical difference in the incidence of adverse events between the two groups(P>0.05).Conclu‐sion:NIPPV can effectively improve blood gas analysis and pulmonary function indexes of COPD patients,shorten the length of hospital stay,and will not increase the incidence of adverse events.
