摘要
目的探讨内支撑技术对膝关节前交叉韧带重建术后患膝早期功能恢复的影响。方法回顾性分析2020年3月—2022年3月中国人民解放军北部战区总医院骨科收治的65例因运动损伤行前交叉韧带重建术患者的临床资料,根据术中编织肌腱是否加入内支撑物分为对照组(移植物为自体肌腱,n=39)和内支撑组(在对照组的基础上增加内支撑物,n=26)。均为男性;年龄18~34岁,平均24.7岁;随访时间12~14个月,平均12.6个月;均为高强度运动致前交叉韧带损伤。记录术后并发症及采用Lysholm膝关节功能评分、国际膝关节评分委员会(IKDC)评分、运动能力评价表(Tegner)进行疗效评估。结果两组患者围术期均未见神经血管损伤、感染、静脉血栓等并发症。术后随访示两组患者术后膝关节功能均较术前改善(P<0.05);术后3个月,内支撑组Lysholm评分为(62.4±4.9)分,IKDC评分为(65.0±4.8)分,Tegner评分为(4.9±0.6)分,均优于对照组[(58.7±3.9)分、(61.6±6.6)分、(4.3±0.4)分],差异有统计学意义(P<0.05);术后12个月两组Lysholm评分、IKDC评分、Tegner评分比较,差异无统计学意义(P>0.05)。结论内支撑技术作为一种提升移植物强度的辅助技术,能够在早期帮助患者快速恢复运动功能、改善患者膝关节功能,值得推广。
Objective To investigate the effect of internal stabilization technique on early functional recovery of anterior cruciate ligament(ACL)injuries after reconstruction.Methods A retrospective analysis was performed on 65 cases of ACL sports injuries that received reconstruction at the Department of Orthopaedics,General Hospital of the Northern Theater Command from Mar.2020 to Mar.2022.Patients were divided into control group with autologous tendon graft(n=39)and internal support group(internal stabilization technique was additionally performed based on autologous tendon graft,n=26).All the patients were males aged 18-34 years,with an average age of 24.7 years,due to high-intensity sports or training.The follow-up period ranged from 12 to 14 months,mean 12.6 months.Postoperative complications were recorded and Lysholm knee function scores,International Knee Documentation Committee(IKDC)scores,and Tegner activity scale(TAS)scores were used to evaluate the outcomes.Results During the perioperative period,no neurovascular injuries,infection,venous thrombosis or other complications were observed in both groups,with significantly improved knee joint function after surgery(both P<0.05 compared to that before surgery).At postoperative 3 months,the internal support group showed much better scores than the control group,i.e.,Lysholm score:62.4±4.9 vs.58.7±3.9,IKDC score:65.0±4.8 vs.61.6±6.6,and Tegner score:4.9±0.6 vs.4.3±0.4,all P<0.05,but all the scores showed no significant difference at 12 months(all P>0.05).Conclusion As an auxiliary technology to improve the strength of grafts,the internal stabilization technique can help patients quickly recover motor function and then improve the knee joint function in the early stage,and thus is worth promoting.
