PLR和DD/Fib预测乳腺癌不良预后价值

摘要 目的:分析血小板与淋巴细胞比值(PLR)和D-二聚体与纤维蛋白原比值(DD/Fib)对乳腺癌患者预后不良的预测价值。方法:选本院2020年1月至2022年5月收入的乳腺癌患者100例为研究对象,根据其术后12个月复查情况将其分为预后良好组(85例)、预后不良组(出现转移、复发、死亡,15例),于所有患者入院时检测血小板与淋巴细胞、D-二聚体与纤维蛋白原水平,计算PLR、DD/Fib。对比患者组间资料分析存在差异指标,采用二元logistics回归分析影响乳腺癌患者预后的因素,对各指标预测预后的价值应通过建立受试者特征工作(ROC)曲线来进行评估。结果:与预后良好组相比,预后不良组患者年龄更高、病程更长、肿瘤直径更大、雌激素受体以及孕激素受体阴性占比更多(P<0.05)。与预后良好组相比,预后不良组PLR、DD/Fib值明显更高(P<0.05)。Logistic回归分析显示肿瘤直径、雌激素受体、孕激素受体、病程、PLR、DD/Fib是影响乳腺癌患者预后不良的因素,其中,肿瘤直径<2cm是乳腺癌患者预后不良的保护因素,雌激素受体阴性、孕激素受体阴性、年龄、病程以及PLR、DD/Fib升高会增加乳腺癌患者预后不良风险(P<0.05)。PLR、DD/Fib单独预测预后不良ROC为0.809、0.761,联合预测预后不良ROC为0.811,三者cut-off值分别为111.07、0.725、0.285。结论:PLR、DD/Fib升高是乳腺癌患者预后不良的危险因素,二者单独、联合预测预后不良均有一定临床参考价值。 Objective:To analyze the predictive value of platelet lymphocyte ratio(PLR)and D-dimer fibrinogen ratio(DD/Lib)for poor prognosis of breast cancer patients.Methods:100 breast cancer patients from January 2020 to May 2022 in our hospital were selected as the research objects.According to the reexamination results 12 months after surgery,they were divided into a good prognosis group(85 cases)and a poor prognosis group(15 cases with metastasis,recurrence and death).When all patients were admitted to the hospital,the levels of blood platelets and lymphocytes,D-dimer and fibrinogen were measured,and PLR,DD/Feb were calculated.There are different indicators in the data analysis between the patient groups.The binary logistic regression analysis was used to analyze the factors affecting the prognosis of breast cancer patients,The value of each indicator should be assessed by establishing a subject characteristic Work(ROC)curve.Result:Compared with the group with good prognosis,patients with poor prognosis were older,had a longer course of disease,had a larger tumor diameter,and had a higher proportion of negative estrogen and progesterone receptors(P<0.05).PLR and DD/Fib values in the poor prognosis group were significantly higher than those in the good prognosis group(P<0.05).Logistic regression analysis showed that tumor diameter,estrogen receptor,progesterone receptor,disease course,PLR,DD/Fib were the factors that affected the poor prognosis of breast cancer patients.Among them,tumor diameter<2cm was the protective factor for poor prognosis of breast cancer patients.Estrogen receptor-negative,progesterone receptor-negative,age and course of disease,PLR,DD/Fib increased the risk of poor prognosis in breast cancer patients(P<0.05).The poor prognostic ROC predicted by PLR and DD/Fib alone was 0.809 and 0.761,while the combined prognostic ROC was 0.811,and the cut-off values of the three were 111.07,0.725 and 0.285,respectively.Conclusion:Elevated PLR and DD/Lib are risk factors for poor prognosis of breast cancer patients.Both alone and jointly predict poor prognosis have certain clinical reference value.
