摘要
目的:运用数据挖掘的方法分析和探讨海派名老中医周华教授治疗冠心病的组方用药规律和学术思想,为冠心病的中药临床运用提供参考。方法:收集2022年9月1日—2023年8月31日周华教授诊疗冠心病的病案,通过中医传承计算平台V 3.0,对中药功效类别和频次、性味及归经进行统计分析,采用关联规则和聚类算法分析核心药物组合。结果:纳入符合研究标准的中药处方105首,包含164味中药,活血化瘀类、补虚类、安神类居多,四气以温性为主,五味以苦味、甘味为主,中药归经以肝、心、肺、脾经居多;用药关联规则网络拓扑图主要包括川芎、丹参、薤白、瓜蒌皮、珍珠母、远志、麦冬、夏枯草;聚类分析提取核心药物组合4组。结论:周华教授治疗冠心病的组方用药规律体现了“标本同治”“心肾同治”“双心同治”的学术思想,为冠心病的中药临床运用提供思路。
Objective:To analyze the prescription and medication rules of Professor Zhou Hua,a famous Traditional Chinese Medicine(TCM)doctor in Shanghai,for the treatment of coronary heart disease(CHD)by data mining method,to explore the academic ideas,and provide reference for the clinical application of TCM for CHD.Methods:The medical records of Professor Zhou Hua's diagnosis and treatment of coronary heart disease from September 1,2022 to August 31,2023 were collected.The TCM inheritance computing platform V3.0 was used to analyze the categories and frequencies,properties,flavors,and meridians-tropism of TCM efficacies.The core drug combinations were analyzed using association rules and clustering algorithms.Results:A total of 105 TCM prescriptions were included,including 164 herbs,most of which were used to promote blood circulation and dissipate blood stasis,with tonifying deficiency,and calming the mind.Most of the four qi prescriptions were warm drugs,and most of the five tastes were bitter and sweet.The meridians of traditional Chinese medicine mostly retated the liver,heart,lung,and spleen meridians.The network topology of drug association rules mainly included Chuanxiong,Danshen,Xiebai,Gualoupi,Zhenzhumu,Yuanzhi,Maidong,and Xiakucao.Four core drug combinations were extracted by cluster analysis.Conclusion:This study analyzes Professor Zhou Hua's prescription and medication rules for the treatment of CHD,reflecting the academic ideas of"treating both symptoms and psychological"and"treating both heart and kidney".This study provides some ideas for the clinical application of TCM for CHD.
