摘要
目的分析当归芍药散联合Bakri球囊填塞术在剖宫产产妇产后大出血中的应用效果。方法回顾性分析2022年1月1日至2024年12月31日项城市卫校中西医结合医院收治的100例剖宫产术后大出血产妇的临床资料,根据治疗措施不同将其分为常规组(50例,Bakri球囊填塞术)和观察组(50例,当归芍药散联合Bakri球囊填塞术)。比较两组的治疗效果。结果两组的术后2 h出血量比较,差异无统计学意义(P>0.05);观察组的术后24 h出血量少于常规组,止血时间、住院时间、恶露持续时间及月经复潮时间短于常规组(P<0.05)。两组的子宫切除率比较,差异无统计学意义(P>0.05);观察组的宫腔粘连、产褥感染发生率低于常规组(P<0.05)。治疗后,观察组的子宫螺动脉收缩期峰值流速(PSV)、舒张末期血流速度(EDV)高于常规组,搏动指数(PI)、阻力指数(RI)低于常规组(P<0.05)。两组术后7 d的子宫体积比较,差异无统计学意义(P>0.05);观察组术后14、30、60 d的子宫体积小于常规组(P<0.05)。结论当归芍药散联合Bakri球囊填塞术在剖宫产产妇产后大出血中的应用效果显著,可快速止血,缩短产后恢复进程,减少产后不良事件,且能调节子宫动脉血流参数,加快产后子宫复旧。
Objective To analyze the application effect of Danggui Shaoyao powder combined with Bakri balloon tamponade in postpartum hemorrhage of cesarean section parturients.Methods The clinical data of 100 parturients with massive hemorrhage after cesarean section admitted in Xiangcheng Health School Hospital of Integrated Traditional Chinese and Western Medicine from January 1,2022 to December 31,2024 were retrospectively analyzed.According to different treatment measures,the parturients were divided into conventional group(50 cases,Bakri balloon tamponade)and observation group(50 cases,Danggui Shaoyao powder combined with Bakri balloon tamponade).The therapeutic effects of the two groups were compared.Results There was no significant difference in the amount of bleeding at 2 h after operation between the two groups(P>0.05);the amount of bleeding at 24 h after operation in the observation group was less than that in the conventional group,and the hemostasis time,hospitalization time,duration of lochia and menstrual recovery time were shorter than those in the conventional group(P<0.05).There was no significant difference in hysterectomy rate between the two groups(P>0.05);the incidence of intrauterine adhesions and puerperal infection in the observation group were lower than thaose in the conventional group(P<0.05).After treatment,peak systolic velocity(PSV)and end-diastolic velocity(EDV)of uterine spiral atrery in the observation group were higher than those in the conventional group,and pulse index(PI)and resistance index(RI)were lower than those in the conventional group(P<0.05).There was no significant difference in uterine volume between the two groups at 7 d after operation(P>0.05);the uterine volume of the observation group was smaller than that of the conventional group at 14,30 and 60 d after operation(P<0.05).Conclusion The Danggui Shaoyao powder combined with Bakri balloon tamponade applied in postpartum hemorrhage of cesarean section parturients has significant effect.It can quickly stop bleeding,shorten the postpartum recovery process,reduce postpartum adverse events,regulate uterine artery blood flow parameters,and accelerate postpartum uterine involution.
