单光子发射计算机断层成像/计算机断层成像复合系统对肿瘤骨转移类型诊断增益价值的研究

摘要 目的 评估单光子发射计算机断层成像/计算机断层成像复合系统(single photon emission computed tomography/computed tomograph,SPECT/CT)在不同类型肿瘤骨转移诊断中的应用价值。方法选取2023年6月—2024年12月阿克苏地区第一人民医院收治的共300例经活检确诊恶性肿瘤伴骨转移患者,全部患者进行SPECT/CT骨显像检查,以病理检查结果为金标准,分析SPECT/CT在不同类型肿瘤骨转移诊断中的灵敏度、特异度、阳性预测值和阴性预测值。结果 经病理检查共发现骨转移971处,其中溶骨性骨转移247处,成骨性骨转移560处,混合性骨转移164处。SPECT/CT骨断层融合显像检查共检出875处骨转移病灶,包括溶骨性骨转移214处,成骨性骨转移521处,混合性骨转移140处。SPECT/CT在不同类型肿瘤骨转移的诊断中表现出较高的灵敏度、特异度、准确度、阳性预测值、阴性预测值。结论 SPECT/CT在早期识别和定位肿瘤骨转移方面具明显优势。 Objective To evaluate the application value of single photon emission computed tomography/computed tomograph(SPECT/CT)in the diagnosis of different tumor bone metastasis type.Methods From June 2023 to December 2024,300 patients with malignant tumor and bone metastasis diagnosed by biopsy in the First People's Hospital of Aksu Region were selected.All patients underwent SPECT/CT bone imaging.The sensitivity,specificity,positive and negative predictive values of SPECT/CT in the diagnosis of different tumor bone metastasis type were analyzed with pathological examination results as the gold standard.Results A total of 971 bone metastases were found by pathological examination,including 247 osteolytic bone metastases,560 osteo⁃genic bone metastases,and 164 mixed bone metastases.SPECT/CT bone fusion imaging,a total of 875 bone metastases were detected,including 214 osteolytic bone metastases,521 osteogenic bone metastases,and 140 mixed bone metastases.SPECT/CT showed high sensitivity,specificity,accuracy,positive predictive value,and negative predictive value in the diagnosis of different tumor bone metastasis type.Conclusion SPECT/CT has obvious advantages in early identification and localization of tumor bone metastasis.
