糖尿病早期肾损伤中尿糖与尿微量白蛋白联合诊断价值分析

摘要 目的探讨糖尿病早期肾损伤中尿糖与尿微量白蛋白联合诊断的价值。方法方法选取2023年6月-2024年6月南充市中心医院收治的80例2型糖尿病患者,以是否有早期肾损伤表现分为单纯糖尿病组(n=40)与糖尿病早期肾损伤组(n=40),并另选同期健康体检者40例作为对照组。分析各组尿糖与尿微量白蛋白的异常情况,采用Pearson相关性分析评估二者与糖尿病早期肾损伤的相关性,并通过绘制受试者工作特征(ROC)曲线分析其单独及联合诊断效能。结果结果糖尿病组的尿糖阳性例数及尿微量白蛋白水平均较健康对照组更高(P<0.05),且糖尿病早期肾损伤组高于单纯糖尿病组(P<0.05)。Pearson相关性结果显示,尿糖(r=0.619)与尿微量白蛋白(r=0.768)水平均与糖尿病早期肾损伤呈正相关(P<0.05)。与单项指标相比,二者联合检测诊断糖尿病早期肾损伤的敏感性与曲线下面积(AUC)更高(P<0.05)。结论结论尿糖联合尿微量白蛋白检测可显著提高糖尿病早期肾损伤的诊断准确性,临床应用价值较高。 Objective To evaluate the diagnostic value of combined urinary glucose and urinary microalbumin testing for the early detection of diabetic nephropathy.Methods A total of 80 patients with type 2 diabetes mellitus(T2DM)admitted to Nanchong Central Hospital from June 2023 to June 2024 were enrolled.Based on the presence or absence of early renal injury,patients were divided into a simple diabetes group(n=40)and an early diabetic nephropathy group(n=40).Additionally,40 healthy individuals undergoing routine physical examinations during the same period were included as a control group.The incidence of abnormal urinary glucose and urinary microalbumin levels was compared among the groups.Pearson correlation analysis was used to assess the association of these markers with early diabetic nephropathy.The diagnostic performance of the individual and combined markers was evaluated using receiver operating characteristic(ROC)curve analysis.Results Both the number of urinary glucose-positive cases and the levels of urinary microalbumin were significantly higher in the diabetic group than in the healthy control group(P<0.05),and were further elevated in the early diabetic nephropathy group compared with the simple diabetes group(P<0.05).Pearson correlation analysis showed a positive correlation between early diabetic nephropathy and both urinary glucose(r=0.619)and urinary microalbumin(r=0.768)(P<0.05).Compared with individual markers,combined testing yielded higher sensitivity and a greater area under the ROC curve(AUC)(P<0.05).Conclusion The combined detection of urinary glucose and urinary microalbumin significantly improves the diagnostic accuracy for early diabetic nephropathy and demonstrates substantial clinical value.
