摘要
目的分析胫骨假体旋转与膝关节单髁置换术(unicompartmental knee arthroplasty,UKA)术后功能的相关性。方法2位研究人员在2023年3月20日分别使用Embase、PubMed和Cochrane图书馆数据库进行文献检索。随后,根据预定的纳入和排除标准对所有检索记录进行独立筛选。使用STROBE声明对文献质量进行评估,对纳入文献中的胫骨假体旋转角度与膝关节功能评分间的相关系数进行数据提取,并应用Fisher’s Z方法进行数据转换,使用Review Manager 5.4软件进行Meta分析。结果共纳入8个原始研究。这些研究根据是否将胫骨假体内、外旋单独讨论进行分类。结果显示,在包含胫骨假体内旋和外旋的研究中,旋转与功能之间没有统计学显著关联(r=-0.06;95%CI:-0.18~0.07;P=0.37)。同样,在仅讨论假体外旋的研究中,胫骨假体外旋与术后功能间也未发现显著的相关性(r=0.04;95%CI:-0.04~0.12;P=0.34)。此外,4篇文献分析了胫骨假体与股骨假体间的相对角度(the tibial component internal rotational angle relative to the femoral component)(简称TIR角)。在包括TIR内旋和外旋的研究中,TIR外旋角度的增加与术后功能存在弱正相关性(r=0.19;95%CI:0.05~0.33;P=0.02);而在仅包含TIR外旋的研究中,TIR外旋角度的增加与术后功能存在显著的负相关(r=-0.54;95%CI:-0.59~-0.48;P<0.001)。结论胫骨假体旋转与短期功能之间不存在相关性。然而,TIR角与短期功能之间存在相关性。
Objective To investigate the correlation between tibial component rotation and function after unicompartmental knee arthroplasty(UKA).Methods Two researchers independently conducted a literature search using Embase,PubMed,and the Cochrane Library for Systematic Reviews on March 20,2023.Subsequently,they independently screened all records based on predefined inclusion and exclusion criteria.The quality of the literature was assessed using the Strengthening the Reporting of Observational Studies in Epidemiology(STROBE)statement.The correlation coefficient between the tibial component rotation angle and knee joint function scores was extracted from the included studies,and Fisher’s Z transformation was applied for data conversion.A meta-analysis was performed using Review Manager 5.4 software.Results A total of eight original studies were identified.These studies were categorized based on whether they discussed internal or external rotation separately.The results indicated that in studies including both internal and external rotation of the tibial component,no statistically significant correlation was found between rotation and function(r=-0.06;95%CI:-0.18-0.07;P=0.37).Similarly,in studies discussing only the external rotation of the tibial component,no significant correlation was found between tibial component external rotation and postoperative function(r=0.04;95%CI:-0.04-0.12;P=0.34).Additionally,four studies analyzed the tibial component internal rotational angle relative to the femoral component(TIR angle).In studies including both TIR internal and external rotation,an increase in TIR external rotation was weakly positively correlated with postoperative function(r=0.19;95%CI:0.05-0.33;P=0.02).However,in studies that only included TIR external rotation,an increase in TIR external rotation was significantly negatively correlated with postoperative function(r=-0.54;95%CI:-0.59-0.48;P<0.001).Conclusions There is no correlation between tibial component rotation and short-term function.However,a correlation is observed between the TIR angle and short-term function after UKA.
