摘要
目的探讨尼尔·诺丁斯关怀理论指导下全程护理对半月板损伤行关节镜修复术患者的影响,并观察其康复效果。方法选取2021年1月—2023年12月徐州医科大学附属医院东院运动医学科接收的86例半月板损伤行关节镜修复术患者,按照组间基线资料均衡可比的原则分为观察组和对照组,每组43例。观察组患者接受尼尔·诺丁斯关怀理论指导下全程护理,对照组患者接受常规护理,护理周期为3个月。比较两组患者膝关节功能[膝关节特种外科医院量表(HSS)评分]、疼痛程度[疼痛强度(PPI)、疼痛分级指数(PRI)及视觉模拟评分法(VAS)]、自我效能[一般自我效能感量表(GSES)]及应对方式[简易应对方式问卷(SCSQ)]。结果护理3个月后,观察组患者HSS评分中疼痛、肌力、活动度、稳定性、关节功能及屈曲畸形得分均高于对照组(P<0.05);观察组患者PPI、PRI、VAS评分均低于对照组(P<0.05);观察组患者GSES评分中健康知识、自我概念、自护技能、自护责任感得分均高于对照组(P<0.05);观察组患者SCSQ评分中积极应对得分高于对照组,消极应对得分低于对照组(P<0.05)。结论尼尔·诺丁斯关怀理论指导下全程护理可改善半月板损伤行关节镜修复术患者膝关节功能,降低疼痛程度,提高自我效能,有效促进应对方式。
Objective To explore the effect of whole-process nursing whole-course nursing guided by Nel Noddings'ethics of care on patients with meniscus injury undergoing arthroscopic repair surgery,and observe its rehabilitation effects.Methods A total of 86 patients who underwent arthroscopic meniscus repair surgery for meniscus injury at the Sports Medicine Department of the Affiliated Hospital of Xuzhou Medical University(east division),from January 2021 to December 2023 were selected.According to the principle of balanced and comparable baseline data between groups,the patients were divided into an observation group and a control group,with 43 patients in each group.The observation group received whole-course nursing guided by Nel Noddings'ethics of care,while the control group received routine nursing care.The nursing cycle lasted for 3 months.The knee joint function[Hospital for Special Surgery Knee Scale(HSS)],pain severity[Present Pain Intensity(PPI),Pain Rating Index(PRI)and Visual Analogue Scale(VAS)],self-efficacy[General Self-Efficacy Scale(GSES)],and coping strategies[Simplifi ed Coping Style Questionnaire(SCSQ)]were compared between the two groups of patients.Results After 3 months of nursing,the observation group had higher scores than the control group in terms of pain,muscle strength,range of motion,stability,joint function,and flexion deformity in the HSS(P<0.05).The observation group also showed higher scores in terms of health knowledge,self-concept,self-care skills,and self-care responsibility in the GSES compared to the control group(P<0.05).The scores of PPI,PRI and VAS in the observation group were lower than those in the control group(P<0.05).Additionally,the observation group had higher scores in positive coping and lower scores in negative coping in the SCSQ compared to the control group(P<0.05).Conclusion Whole-course nursing guided by Nel Noddings'ethics of care can improve knee joint function,reduce pain levels,enhance self-efficacy,and effectively promote coping strategies in patients with meniscus injury undergoing arthroscopic repair surgery.
