摘要
目的:分析条口透承山针刺联合体外冲击波治疗粘连前期肩周炎的效果。方法:收集2023年4月—2024年1月漯河市中心医院疼痛科收治的95例粘连前期肩周炎患者的临床资料,依照治疗方案不同分为2组,对照组(n=46)接受体外冲击波治疗,观察组(n=49)接受条口透承山针刺联合体外冲击波治疗。疗程结束后,比较2组疗效、治疗前后肩关节活动度(外旋、前屈、外展、后伸、内旋)、疼痛程度(McGill疼痛量表)、血清炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)、白细胞介素-6(IL-6)]水平。结果:与对照组(80.43%)比较,观察组总有效率(95.92%)更高(P<0.05);治疗后,2组疼痛程度均减轻,且观察组McGill疼痛量表总分及各分量表评分均低于对照组(P<0.05);治疗后,2组肩关节外旋、外展、内旋、前屈、后伸活动度均较治疗前增加,且与对照组比较,观察组肩关节外旋、外展、内旋、前屈、后伸活动度均更大(P<0.05);治疗后,2组血清TNF-α、IL-6水平均降低,IL-10水平均升高,且观察组血清TNF-α、IL-6水平较对照组低,IL-10水平较对照组高(P<0.05)。结论:条口透承山针刺联合体外冲击波治疗粘连前期肩周炎的效果显著,能有效减轻患者疼痛程度、改善肩关节活动度、调节血清炎性因子水平。
Objective:To analyze the effect of penetration acupuncture from Tiaokou to Chengshan acupoints combined with extracorporeal shock wave therapy in the treatment of pre-adhesive frozen shoulder.Methods:The clinical data of 95 patients with pre-adhesive frozen shoulder in the department of pain of Luohe Central Hospital from April 2023 to January 2024 were collected,and they were divided into two groups according to different treatment regimens.The control group(n=46)was received extracorporeal shock wave therapy,and the observation group(n=49)was received penetration acupuncture from Tiaokou to Chengshan acupoints combined with extracorporeal shock wave therapy.After the course of treatment,the efficacy was compared between the two groups,as well as the shoulder joint range of motion(external rotation,forward flexion,abduction,extension,internal rotation),pain level(McGill Pain Questionnaire),and serum inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-10(IL-10),and interleukin-6(IL-6)]before and after treatment.Results:Compared with the control group(80.43%),the total effective rate of the observation group(95.92%)was higher(P<0.05).After treatment,the pain levels in both groups were reduced,and the total score and subscale scores of the McGill Pain Questionnaire in the observation group were lower than those in the control group(P<0.05).After treatment,the shoulder joint range of motion in both groups was increased compared with before treatment,and the shoulder joint range of motion in the observation group was greater than that in the control group(P<0.05).After treatment,the levels of TNF-αand IL-6 in both groups were decreased,while the level of IL-10 was increased.The levels of TNF-αand IL-6 in the observation group were lower than those in the control group,while the level of IL-10 was higher than that in the control group(P<0.05).Conclusions:Penetration acupuncture from Tiaokou to Chengshan acupoints combined with extracorporeal shock wave in the treatment of pre-adhesive frozen shoulder has a significant effect,which can effectively reduce the pain level,improve the range of motion of the shoulder joint,and regulate the level of serum inflammatory factors.
