脑血流超声参数、血清sLOX-1水平与新生儿HIE病情程度的相关性分析及临床意义

Correlation analysis and clinical significance of cerebral blood flow ultrasound,serum sLOX-1 and the severity of neonatal hypoxic-ischemic encephalopathy in infants

摘要 目的探讨脑血流超声参数、血清可溶性凝集素样氧化低密度脂蛋白受体-1(sLOX-1)水平与新生儿缺氧缺血性脑病(HIE)病情程度的相关性及临床意义。方法回顾性选取133例HIE新生儿为观察组,依据1∶1匹配原则,选取同期出生的健康足月新生儿133例为对照组,比较2组新生儿脑血流超声参数[收缩期峰值流速(Vs)、舒张期流速(Vd)、平均流速(Vm)、阻力指数(RI)]及血清sLOX-1水平并分析相关性,比较不同预后新生儿脑血流超声参数(Vs、Vd、Vm、RI)、血清sLOX-1水平并分析各指标联合检测对新生儿预后的预测价值。结果观察组预后不良新生儿Vs、RI、sLOX-1值水平小于预后良好新生儿,Vd、Vm值大于预后良好新生儿(P<0.05);病情程度重度新生儿Vs、RI、sLOX-1值低于中度新生儿与轻度新生儿,Vd、Vm值高于中度新生儿与轻度新生儿(P<0.05),脑血流超声参数(Vs、Vd、Vm、RI)联合血清sLOX-1水平预测新生儿预后的AUC为0.926,约登指数为0.800,敏感度为0.944,特异度为0.856。结论脑血流超声参数、血清sLOX-1水平与HIE新生儿病情程度关系密切,二者联合可为临床预测HIE新生儿预后提供一定参考价值。 Objective To explore the correlation and clinical significance of cerebral blood flow parameters by ultrasound and serum soluble lectin-like oxidized low-density lipoprotein receptor-1(sLOX-1)levels with the severity of neonatal hypoxic ischemic encephalopathy(HIE).Methods A total of 133 HIE newborns were selected as the observation group.According to the principle of 1:1 selection,133 healthy full-term newborns born in the same period were selected as the control group.The ultrasound parameters of cerebral blood flow included systolic peak velocity(Vs),diastolic velocity(Vd),mean velocity(Vm),and resistance index(RI)and serum sLOX-1 levels were compared between the two groups and children with different disease severity levels,and the correlation was analyzed.The ultrasound parameters(Vs,Vd,Vm,RI)of cerebral blood flow and serum sLOX-1 levels were compared in children with different prognoses,and the predictive value of combined detection of these indicators was analyzed for the prognosis of children.Results The Vs,RI,and sLOX-1 values in the observation group and children with poor prognosis were lower than those in the control group and children with good prognosis,while the Vd,Vm value were higher than those in the control group and children with good prognosis(P<0.05).The Vs,RI and sLOX-1 values of children with severe disease were less than those of children with moderate disease and those of children with mild disease,and the Vd,Vm value were more than those of children with moderate disease and those of children with mild disease(P<0.05).The AUC of cerebral blood flow ultrasound parameters(Vs,Vd,Vm,RI)combined with serum sLOX-1 level for predicting the prognosis of children was 0.926,with Youden index of 0.800,sensitivity of 0.944,and specificity of 0.856.Conclusion The ultrasound parameters of cerebral blood flow and the level of serum sLOX-1 are closely related to the severity of HIE newborns.The combination of ultrasound parameters of cerebral blood flow and serum sLOX-1 can provide some reference value for clinical prediction of the prognosis of HIE in children.
