全自动生化分析仪联合尿液分析仪在2型糖尿病诊断及病情评估中的应用

摘要 目的分析全自动生化分析仪联合尿液分析仪在2型糖尿病诊断及病情评估中的应用价值。方法回顾性选取2023年12月至2024年6月在我院体检的140例疑似2型糖尿病患者作为研究对象,均利用CA-800C型全自动生化分析仪、US-2000PH型全自动尿液分析仪分别检测血糖、尿糖水平,以口服葡萄糖耐量试验结果作为金标准,分析全自动生化分析仪、尿液分析仪单一与联合检测对2型糖尿病的诊断效能(灵敏度、特异度、准确度)。依据2型糖尿病阳性患者的病情(是否并发肾病)分为糖尿病肾病组、非糖尿病肾病组,对比两组全自动生化分析仪检出的血糖指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)]及全自动尿液分析仪检出的尿酸(UA)、尿素氮(BUN)水平。绘制受试者工作特征(ROC)曲线分析全自动生化分析仪、尿液分析仪单独及联合检测对2型糖尿病患者病情的评估价值。结果140例疑似2型糖尿病患者金标准检出阳性84例、阴性56例;全自动生化分析仪、尿液分析仪联合检出阳性83例(含真阳性81例)、阴性57例(含真阴性54例);联合检测诊断2型糖尿病的灵敏度、特异度、准确度均较单一检测高(P<0.05)。糖尿病肾病组的FPG、2hPG、HbA1c水平以及UA、BUN水平均较非糖尿病肾病组高(P<0.05)。ROC曲线显示,全自动生化分析仪、尿液分析仪联合检测评估2型糖尿病病情的曲线下面积(AUC)为0.930,较单一检测高。结论全自动生化分析仪联合尿液分析仪在2型糖尿病诊断及病情评估中应用价值较高,其分别检测的FPG、2hPG、HbA1c水平与UA、BUN水平可作为临床评估2型糖尿病并发肾病的常用指标,但仍需结合患者实际情况综合考量。 Objective To analyze the application value of fully automatic biochemical analyzer combined with urine analyzer in the diagnosis and disease evaluation of type 2 diabetes mellitus.Methods 140 patients with suspected type 2 diabetes mellitus who underwent physical examination in our hospital from December 2023 to June 2024 were retrospectively selected as the research objects,and the levels of blood sugar and urine sugar were tested by CA-800 C fully automatic biochemical analyzer and US-2000PH automatic urine analyzer.The results of oral glucose tolerance test were used as the gold standard to analyze the diagnostic efficacy(sensitivity,specificity,accuracy)of single and combined detection of fully automatic biochemical analyzer and urine analyzer for type 2 diabetes mellitus.According to the disease condition of patients with type 2 diabetes mellitus(whether complicated with nephropathy),they were divided into the diabetic nephropathy group and the non-diabetic nephropathy group,and the blood glucose indicators[fasting blood glucose(FPG),2 h postprandial blood glucose(2hPG),glycosylated hemoglobin(HbAlc)]detected by fully automatic biochemical analyzer and uric acid(UA),urea nitrogen(BUN)detected by automatic urine analyzer were compared between the two groups.The receiver operating characteristic(ROC)curve was drawn to analyze the evaluation value of single and combined detection of fully automatic biochemical analyzer and urine analyzer for patients with type 2 diabetes mellitus.Results Among 140 patients with suspected type 2 diabetes mellitus,84 cases were positive and 56 cases were negative according to the gold standard test.83 positive cases(including 81 true positive cases)and 57 negative cases(including 54 true negative cases)were detected by fully automatic biochemical analyzer combined with urine analyzer.The sensitivity,specificity and accuracy of combined detection in the diagnosis of type 2 diabetes mellitus were higher than those of single detection(P<0.05).The levels of FPG,2hPG,HbAlc,UA,BUN in the diabetic nephropathy group were higher than those in the non-diabetic nephropathy group(P<0.05).ROC curve showed that the area under the curve(AUC)of the combined detection of fully automatic biochemical analyzer and urine analyzer to evaluate the disease condition of type 2 diabetes mellitus was 0.930,higher than that of single detection.Conclusions The application value of fully automatic biochemical analyzer combined with urine analyzer in the diagnosis and disease evaluation of type 2 diabetes mellitus is high,the levels of FPG,2hPG,HbAlc,and UA and BUN detected by fully automatic biochemical analyzer combined with urine analyzer respectively can be used as common indicators for clinical evaluation of type 2 diabetes mellitus complicated with nephropathy,but it still needs to be considered comprehensively in combination with the actual situation of patients.
