摘要
目的探讨直接喉镜联合可视导芯在急诊气管插管教学中的应用效果。方法选取2022年7月至2024年6月于南宁市第二人民医院急诊科行临床轮转的120名学员作为研究对象,根据不同的辅助工具使用情况分为直接喉镜组(A组)、可视喉镜组(B组)、直接喉镜组+可视导芯组(C组),每组40名。所有学员均采用相同教材,分批次进行气管插管理论教学,均接受直接喉镜气管插管、可视喉镜气管插管、直接喉镜+可视导芯气管插管培训,A组使用可视喉镜进行气管插管实操教学,B组使用可视导芯喉镜进行气管插管实操教学,C组使用直接喉镜+可视导芯喉镜进行实操教学。比较3组气管插管自信度、兴趣度、气管插管考核指标(气管插管成功所需时间、成功气管插管所需次数、一次插管成功率和气管插管技能考核成绩)。结果培训后,B组、C组自信度评分均高于A组,且C组高于B组,差异有统计学意义(P<0.05)。培训后,B组、C组兴趣度评分均高于A组,且C组高于B组,差异有统计学意义(P<0.05)。B组、C组成功气管插管所需时间均短于A组,成功插管所需次数均少于A组,差异有统计学意义(P<0.05),B组与C组成功气管插管所需时间、成功插管所需次数比较差异无统计学意义。B组、C组一次插管成功率、气管插管技能考核成绩均高于A组,且C组高于B组,差异有统计学意义(P<0.05)。结论与使用直接喉镜和可视喉镜实操教学相比,直接喉镜联合可视导芯在急诊气管插管教学中的应用效果更好,可提高学员气管插管自信度、兴趣度、一次插管成功率、气管插管技能考核成绩,缩短成功气管插管所需时间,减少成功插管所需次数。
Objective To explore the application effect of direct laryngoscope combined with visual guide core in the teaching of emergency endotracheal intubation.Methods A total of 120 trainees undergoing clinical rotation in the Emergency department of the Second People's Hospital of Nanning from July 2022 to June 2024 were selected as research subjects,and they were divided into the direct laryngoscope group(group A),video laryngoscope group(group B),and direct laryngoscope+visual guide core group(group C)according to the usage of different auxiliary tools,with 40 trainees in each group.All trainees used the same textbook and underwent theoretical teaching on tracheal intubation in batches.They received training in direct laryngoscopy,video laryngoscopy,and direct laryngoscopy+visual guide core tracheal intubation,the group A used video laryngoscopy for practical teaching of tracheal intubation,the group B used visual guide core laryngoscopy for practical teaching of tracheal intubation,and the group C used direct laryngoscope+visual guide core laryngoscopy for practical teaching.The confidence level,interest level,and tracheal intubation assessment indicators(time required for successful tracheal intubation,number of attempts required for successful intubation,first-attempt success rate,and tracheal intubation skill assessment score)were compared among the three groups.Results After training,the confidence scores in the group B and group C were higher than that in the group A,and the group C was higher than the group B,the differences were statistically significant(P<0.05).After training,the interest scores in the group B and group C were higher than those in the group A,and the group C was higher than the group B,the differences were statistically significant(P<0.05).The time required for successful tracheal intubation in the group B and group C were shorter than that in the group A,and the number of attempts required for successful intubation were less that in the group A,the differences were statistically significant(P<0.05),there were no statistically significant differences in the time required for successful tracheal intubation,the number of attempts required for successful intubation between the group B and the group C.The success rate of one-time intubation and the assessment scores of tracheal intubation skills in the group B and group C were higher than those in the group A,and the group C was higher than the group B,the differences were statistically significant(P<0.05).Conclusion Compared to using direct laryngoscopes and video laryngoscopes for practical teaching,the combination of direct laryngoscope and visual guide core in emergency tracheal intubation teaching has better application effects,improving trainees'confidence,interest,first-attempt success rate,and tracheal intubation skill assessment scores,reducing the time required for successful tracheal intubation,and decreasing the number of attempts required for successful intubation.
