摘要
目的评估视频辅助肛瘘治疗术在经括约肌肛瘘患者临床治疗中的应用价值,为临床术式选择提供参考依据。方法选取2020年4月至2021年4月共83例来自北京市肛肠医院(北京市二龙路医院)复杂性肛瘘患者作为研究对象,采用随机数字表法分组,对照组41例行传统切开挂线术,试验组42例行视频辅助肛瘘治疗术,比较两组的各项手术指标(术中失血量、手术时间、伤口愈合时间及住院时间)、肛门功能评分及术后并发症发生情况。结果试验组各项手术指标(手术时间、伤口愈合时间、住院时间和术中出血量)显著优于对照组(P<0.05);两组患者手术实施前Wexner评分差异无统计学意义(P>0.05),试验组患者术后6个月与术后1年Wexner评分均显著低于对照组(P<0.05);与对照组比,试验组术后并发症总体发生率显著降低(P<0.05)。结论视频辅助肛瘘治疗术用于经括约肌肛瘘患者治疗,手术时间短,术中出血量低,伤口愈合时间及住院时间短,肛门功能恢复良好且术后并发症少。
Objective To evaluate the application value of video-assisted anal fistula treatment in the clinical treatment of patients with anal fistula through sphincter,so as to provide a reference for clinical practitioners to choose.Methods A total of 83 patients with complex anal fistula from the Beijing Anorectal Hospital(Beijing Erlong Road Hospital)were selected as the research objects from April 2020 to April 2021.The patients were randomly divided into the control group(41 cases)and the experimental group(42 cases).The control group was treated with traditional incision and seton,and the experimental group was treated with video-assisted anal fistula treatment.The surgical indicators(intraoperative blood loss,operation time,wound healing time and hospital stay),anal function score and postoperative complications were compared between the two groups.Results The operation indexes(operation time,wound healing time,hospitalization time and intraoperative blood loss)in the experimental group were significantly superior than those of the control group(P<0.05).There was no significant difference in Wexner scores between the two groups before operation(P>0.05).The Wexner scores of the experimental group were significantly lower than those of the control group at 6 months and 1 year after operation(P<0.05).Compared with the control group,the overall incidence of postoperative complications in the experimental group was significantly reduced(P<0.05).Conclusions Video-assisted anal fistula treatment for patients with transsphincteric anal fistula has the advantages of short operation time,low intraoperative blood loss,short wound healing time and hospital stay,good anal function recovery and few postoperative complications.
