基于家用呼吸机的高频振荡排痰系统研究

摘要 针对呼吸道患者气道黏液清除困难,长期治疗费用高等问题,设计了一种基于家用呼吸机的高频振荡排痰系统。在呼吸机正压通气下,该系统通过控制扬声器振荡源的频率、幅值产生周期性振荡气流作用于人体气道,体外模拟小气道黏液栓实验表明:该系统可以通过流体剪切力移动和稀化黏液栓。同时在不影响排痰效果情况下,提出了低幅高频的保护性通气优化策略,规避了高气道压导致的肺部气压伤风险。该系统操作简单,价格便宜,尤其适合中重度慢性阻塞性肺疾病、囊性纤维化等黏液潴留慢性呼吸道疾病患者使用,具有重要的临床应用价值。 Aiming to address the difficulties in airway mucus clearance and high long-term treatment cost in respiratory patients,a high frequency oscillatory sputum excretion system based on home ventilator is designed.Under the positive ventilation pressure of the ventilator,the system controls the frequency and amplitude of the loudspeaker oscillation source to produce periodic oscillating air flow to act on the human airway.In vitro experiment simulation of small airway mucus plugs shows that this system could move and dilute mucus plugs through fluid shear force.At the same time,without affecting the effect of sputum expulsion,a protective ventilation optimization strategy of low amplitude and high frequency is proposed to avoid the risk of pulmonary barotrauma caused by high airway pressure.The system is easy to operate and cheap,especially suitable for chronic obstructive pulmonary disease,cystic fibrosis and other chronic respiratory diseases with mucus retention,and has important clinical application value.
