摘要
目的探究类肺炎性胸腔积液诊疗中彩色多普勒超声的应用。方法选取2023年04月-2024年02月鹤壁市人民医院收治的类肺炎性胸腔积液患者80例为研究对象。根据随机数字表法分为彩色组(彩色多普勒超声,40例)和X线组(X线检查,40例)。比较两组诊断确切率、一次穿刺成功率、穿刺治愈率、超声分型及有效率。结果彩色组诊断确切率显著高于X线组(P<0.05);彩色组一次穿刺成功率显著高于X线组(P<0.05);彩色组穿刺治愈率高于X线组(P<0.05);80例类肺炎性胸腔积液患者的超声分型:Ⅰ型12例,Ⅱ型14例,ⅢA型17例,ⅢB型22例,ⅢC型5例,Ⅳ型10例,总有效率分别为100%、92.86%、94.12%、86.36%、80.00%、80.00%。结论彩色多普勒超声在诊断类肺炎性胸腔积液方面展现出便捷、无创、可重复的优势,其诊断准确性显著优于X线检查,能提高穿刺成功率与治愈率。依据超声分型制定的个性化治疗方案,在临床上取得良好成效。
Objective To explore the application of color Doppler ultrasound in the diagnosis and treatment of pneumonic pleural effusion.Methods From April 2023 to February 2024,80 cases of pneumonic patients admitted to Hebi People's Hospital were selected as the research object.According to the random number table,they were divided into color group(color Doppler ultrasound,40 cases)and X-ray group(X-ray examination,40 cases).The diagnostic accuracy,the success rate of one puncture,the cure rate of puncture,the ultrasonic classification and the effective rate were compared between the two groups.Results The diagnostic accuracy of color group was significantly higher than that of X-ray group(P<0.05).The success rate of one puncture in color group was significantly higher than that in X-ray group(P<0.05).The cure rate of puncture in color group was higher than that in X-ray group(P<0.05).Ultrasonic classification of 80 patients with pneumonic pleural effusion:There were 12 instances of type I,14 of type II,17 categorized as type IIIA,22 as type IIIB,and an additional 5 cases belonging to a different,unspecified typeⅢC and 10 cases of typeⅣ.The total effective rates were 100%,92.86%,94.12%,86.36%,80.00%and 80.00%respectively.Conclusions Color Doppler ultrasound is simple,non-invasive and reproducible in the diagnosis of pneumonic pleural effusion.The positive rate of diagnosis is significantly higher than that of X-ray examination,and the success rate and cure rate of puncture are high.It has good clinical effect to formulate individualized treatment plan according to ultrasonic classification.
