摘要
目的探究单光子发射计算机断层显像(SPECT-CT)联合含有蛋白裂解信号PEST序列的核蛋白(PCNP)水平检测在肺腺癌危险度分级中的应用价值。方法回顾性分析2022年1月至2024年12月河南大学第一附属医院收治并确诊为肺腺癌的患者135例,根据不同检查方法分为对照组(n=67)和观察组(n=68),对照组进行单独SPECT-CT检查,观察组在此基础上联合PCNP水平检测对肺腺癌危险度进行评估。分析两组检查方案对患者肺腺癌危险度分级的准确性,评估两组方案的诊断效能。结果经穿刺活检组织结合患者实际情况,共测得59例高危患者和76例中低危患者,其中观察组诊断正确率95.59%,显著高于对照组79.10%;在高危患者中,SPECT-CT阳性率和PCNP水平显著高于中低危患者(P<0.05);经Logistic回归分析,结果显示SPECT-CT阳性率与PCNP水平均可作为肺腺癌危险度分级的评估指标;经受试者工作特征(ROC)曲线分析,联合诊断所获得的ROC曲线AUC为0.920,高于SPECT-CT阳性(AUC=0.908)与PCNP水平(AUC=0.902)。结论SPECT-CT联合PCNP水平检测在肺腺癌危险度分级中与单独SPCET-CT检测相比,具有较高的准确性和诊断效能。
Objective To explore the application value of single photon emission computed tomography(SPECT-CT)combined with the detection of PEST Proteolytic Signal Containing Nuclear Protein(PCNP)levels in the risk classification of lung adenocarcinoma.Methods A retrospective analysis was conducted on 135 patients diagnosed with lung adenocarcinoma and admitted to the First Affiliated Hospital of Henan University from January 2022 to December 2024.They were divided into a control group(n=67)and an observation group(n=68)according to different examination methods.The control group underwent individual SPECT-CT examination.On this basis,the observation group evaluated the risk of lung adenocarcinoma in combination with the detection of PCNP levels.The accuracy of examination protocols of the two groups in the risk classification of lung adenocarcinoma in patients was analyzed and the diagnostic efficacy of protocols of the two groups was evaluated.Results Through puncture biopsy tissues combined with the actual situation of the patients,a total of 59 high-risk patients and 76 medium and low-risk patients were measured.Among them,the diagnostic accuracy rate of the observation group was 95.59%,significantly higher than that of the control group 79.10%.Among high-risk patients,the positive rate of SPECT-CT and the level of PCNP were significantly higher than those in medium and low-risk patients(P<0.05);Logistic regression analysis showed that both the positive rate of SPECT-CT and the level of PCNP could be used as assessment indicators for the risk classification of lung adenocarcinoma.Through ROC curve analysis,the AUC of the ROC curve obtained by the combined diagnosis was 0.920,which was higher than that of SPECT-CT positive(AUC=0.908)and PCNP level(AUC=0.902).Conclusion The detection of SPECT-CT combined with PCNP level has higher accuracy and diagnostic efficacy compared with the detection of SPC-ET-CT alone in the risk classification of lung adenocarcinoma.
