摘要
目的 分析心脏手术中输入新鲜冰冻血浆(FFP)和红细胞悬液(PRBC)对凝血功能影响。方法 选择2021年6月至2023年6月住院的110例心脏手术输血患者,根据输血方式的不同将其分为3组,40例FFP∶PRBC为1∶1的患者设为试验组,36例FFP:PRBC为1∶2的患者设为对照A组,34例FFP:PRBC为1∶3的患者设为对照B组,比较3组凝血功能[纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、凝血酶原时间(PT)]、电解质平衡指标、输血并发症总发生率、住院时间、死亡率。结果 输血后试验组FIB高于对照A组、对照B组(P<0.05)。输血后试验组APTT、TT、PT均低于对照A组、对照B组(P<0.05),输血后对照A组、对照B组FIB、APTT、TT、PT比较,差异无统计学意义(P>0.05)。输血后试验组Na^(+)、Ca^(2+)均低于对照A组、对照B组(P<0.05),输血后试验组K^(+)高于对照A组、对照B组(P<0.05),输血后对照A组、对照B组Na^(+)、Ca^(2+)、K^(+)比较,差异无统计学意义(P>0.05)。试验组输血并发症总发生率(2.50%)与对照A组(5.56%)、对照B组(5.88%)比较,差异无统计学意义(P>0.05)。试验组住院时间、死亡率(0)与对照A组(2.78%)、对照B组(2.94%)比较,差异无统计学意义(P>0.05)。结论 心脏手术中输入1∶1的FFP:PRBC,可有效改善凝血功能,维持电解质平衡,且未增加并发症发生风险及死亡率,安全有效。
Objective To analyze the effects of infusing fresh frozen plasma(FFP)and packed red blood cell suspension(PRBC)on coagulation function during cardiac surgery.Methods A total of 110 patients with cardiac operation and intraoperative blood transfusion from June 2021 to June 2023 were selected.They were divided into three groups according to different transfusion methods,namely experimental group(FFP∶PRBC=1∶1,n=40),control A group(FFP∶PRBC=1∶2,n=36),and control B group(FFP∶PRBC=1∶3,n=34).Coagulation function(fibrinogen[FIB],activated partial thromboplastin time[APTT],thrombin time[TT],prothrombin time[PT]),electrolyte balance indexes,total incidence of transfusion complications,length of hospital stay,and mortality of the three groups were compared.Results The FIB of the experimental group was significantly higher than that of control group A and control group B after blood transfusion(P<0.05).After blood transfusion,the APTT,TT and PT of the experimental group were significantly lower than those of control group A and control group B(P<0.05),and the FIB,APTT,TT and PT of control group A and control group B were comparable after blood transfusion(P>0.05).After blood transfusion,Na+and Ca 2+in the experimental group were significantly lower than those in control group A and control group B(P<0.05),and K+in the experimental group was significantly higher than that in control group A and control group B(P<0.05).After blood transfusion,Na+,Ca 2+and K+in control group A and control group B were comparable(P>0.05).The total incidence of blood transfusion complications in the experimental group(2.50%)was comparable with that in control group A(5.56%)and control group B(5.88%)(P>0.05).The hospital stay and mortality(0)of the experimental group were comparable with those of control group A(2.78%)and control group B(2.94%)(P>0.05).Conclusion In cardiac surgery,infusion of FFP∶PRBC at 1∶1 can effectively improve coagulation function,maintain electrolyte balance,and does not increase the risk of complications and mortality.It is safe and effective.
