摘要
目的观察名老中医赵炳南经验方“苍肤洗剂”干纸巾外用治疗手足癣的临床疗效及安全性。方法选择2022年1月—2023年12月本院皮肤科门诊收治的手足癣患者144例。采用前瞻性、随机、对照研究,按照随机数字表法将入选病例分为A、B、C组。A组采用苍肤洗剂水煎剂治疗,B组采用苍肤洗剂干纸巾治疗,C组采用特比萘芬乳膏治疗。3组均以1周为1疗程,连续治疗2周。对比3组疗效,治疗前后皮损面积、瘙痒程度评分、皮损严重程度评分,真菌清除率,不良反应,复发率。结果3组总有效率比较,差异无统计学意义(P>0.05)。治疗后,3组皮损面积、瘙痒程度评分、皮损严重程度评分均较治疗前改善(P<0.01)。治疗后,3组间皮损面积、皮损严重程度评分比较,差异有统计学意义(P<0.05);进一步两两比较,A组皮损面积小于B组(P<0.05),A、B组皮损严重程度评分均低于C组(P<0.01)。3组真菌清除率、不良反应发生率比较,差异均无统计学意义(P>0.05)。A组、B组复发率低于C组,但差异无统计学意义(P>0.05)。结论苍肤洗剂外用治疗手足癣疗效确切,且无明显不良反应;干纸巾这一专利技术应用于中药外治,提升了便携度。
Objective To observe the clinical efficacy and safety of the external application of Cangfu Lotion with dry tissues,a traditional Chinese medicine(TCM)formula based on the experience of renowned physician ZHAO Bingnan,in the treatment of tinea manuum and pedis.Methods A prospective,randomized,controlled study was conducted involving 144 patients with tinea manuum and pedis treated at the Dermatology Outpatient Department of Beijing Hospital of Traditional Chinese Medicine from January 2022 to December 2023.Patients were randomly assigned by a random number table to Group A,Group B,or Group C.Group A was treated with Cangfu Lotion decoction soaking,Group B received dry tissues soaked in Cangfu Lotion for daily application,and Group C was treated with topical terbinafine cream.Each group underwent treatment for one week per course,for two consecutive courses.The efficacy was compared among the three groups by evaluating lesion area,itching severity score,lesion severity score,fungal clearance rate,adverse reactions,and recurrence rate before and after treatment.Results There was no statistically significant difference in the total effective rate among the three groups(P>0.05).After treatment,lesion area,itching severity score,and lesion severity score were significantly improved in all three groups compared to those before treatment(P<0.01).Significant differences were found among the three groups in lesion area and lesion severity score after treatment(P<0.05).Pairwise comparison showed that the lesion area in Group A was smaller than that in Group B(P<0.05),and lesion severity scores in Groups A and B were lower than those in Group C(P<0.01).No statistically significant differences were found among the groups in fungal clearance rate or incidence of adverse reactions.Recurrence rates in Groups A and B were lower than in Group C,but the difference was not statistically significant(P>0.05).Conclusion External application of Cangfu Lotion is effective and safe in treating tinea manuum and pedis.The patented dry tissue technology applied in TCM external treatment improves convenience and portability.
