摘要
目的 探讨二甲双胍恩格列净治疗射血分数减低型心力衰竭(heart failure with reduced ejection fraction,HFrEF)合并2型糖尿病(type 2 diabetes mellitus, T2DM)患者的临床疗效。方法 选择2022年8月—2024年8月在烟台芝罘医院接受治疗的80例HFrEF伴T2DM患者,按照不同治疗方式分组,每组40例。对照组接受美托洛尔联合二甲双胍治疗,观察组则接受美托洛尔联合二甲双胍恩格列净的治疗。对比两组血糖水平、心功能、心室重塑水平以及不良反应发生率。结果 治疗后,观察组血糖水平低于对照组,心功能和心室重塑水平优于对照组,差异均有统计学意义(P均<0.05)。观察组不良反应发生率为10.00%(4/40),略低于对照组的12.50%(5/40),但差异无统计学意义(χ^(2)=0.000,P>0.05)。结论 二甲双胍恩格列净联合美托洛尔治疗能有效降低HFrEF合并T2DM患者的血糖水平,改善心功能和心室重塑,且安全性相当。
Objective To investigate the clinical efficacy of metformin and empagliflozin in the treatment of heart fail-ure with reduced ejection fraction(HFrEF)complicated with type 2 diabetes mellitus(T2DM).Methods A total of 80 patients with HFrEF and T2DM who were treated in Yantai Zhifu Hospital from August 2022 to August 2024 were se-lected and divided into groups according to different treatment methods,with 40 patients in each group.The control group was treated with metoprolol combined with metformin,while the observation group was treated with metoprolol combined with metformin empagliflozin.The blood glucose level,cardiac function,ventricular remodeling level and in-cidence of adverse reactions were compared between the two groups.Results After treatment,the blood glucose level in the observation group was lower than that in the control group,and the levels of cardiac function and ventricular re-modeling were better than those in the control group,the differences were statistically significant(all P<0.05).The in-cidence of adverse reactions in the observation group was 10.00%(4/40),which was slightly lower than 12.50%(5/40)in the control group,but the difference was not statistically significant(χ^(2)=0.000,P>0.05).Conclusion Metformin empagliflozin combined with metoprolol can effectively reduce the blood glucose level,improve cardiac function and ventricular remodeling in patients with HFrEF and T2DM,and the safety is comparable.
