摘要
探讨在儿童肺炎支原体感染(mycoplasma pneumoniae,MP)诊断中,采用鼻咽拭子支原体核糖核酸(mycoplasma ribonucleic acid,MP-RNA)联合血清免疫球蛋白M(ImmunoglobulinM,IgM)滴度检测的临床应用价值。选取成都市龙泉驿区妇幼保健院2024年1月—2025年3月接诊的105例高度疑似MP感染患儿作为研究对象,组织进行鼻咽拭子MPRNA、血清IgM滴度标本采集与检测,并根据肺内肺外表现进行临床诊断,整理不同方法的检测结果。结果显示,临床诊断共检出37例阳性患儿,阳性率为35.24%。鼻咽拭子MP-RNA、血清IgM滴度检测阳性率分别为32.38%、28.57%,与临床诊断结果比较,无统计学差异(P>0.05)。以临床诊断结果为参照标准,鼻咽拭子MP-RNA联合血清IgM滴度检测的敏感度、特异度、阳性预测值、阴性预测值分别为97.30%、98.53%、97.30%、98.53%,高于鼻咽拭子MP-RNA、血清IgM滴度单独检测,差异有统计学意义(P<0.05)。研究发现,在MP感染患儿诊断中,鼻咽拭子MP-RNA、血清IgM滴度单独检测具有一定价值,但联合运用可进一步提升诊断敏感度,值得推广。
This study explores the clinical application value of nasopharyngeal swab mycoplasma pneumoniae ribonucleic acid(MP-RNA)combined with serum immunoglobulin M(IgM)titer detection in the diagnosis of mycoplasma pneumoniae(MP)infection in children.A total of 105 children with highly suspected MP infection admitted to Longquanyi District of Chengdu Maternal and Child Health Hospital from January 2024 to March 2025 were enrolled.Nasopharyngeal swab MP-RNA and serum IgM titer were detected,and clinical diagnosis was performed based on intra-pulmonary and extra-pulmonary manifestations to summarize the test results of different methods.Clinical diagnosis identifi ed 37 positive cases,with a positive rate of 35.24%.The positive rates of nasopharyngeal swab MP-RNA and serum IgM titer were 32.38%and 28.57%,respectively,showing no significant difference from clinical diagnosis(P>0.05).Using clinical diagnosis as the reference standard,the combined detection of MP-RNA and IgM titer exhibited sensitivity,specifi city,positive predictive value,and negative predictive value of 97.30%,98.53%,97.30%,and 98.53%,signifi cantly higher than single detections(P<0.05).The study confi rms that although single detection of nasopharyngeal swab MP-RNA or serum IgM titer has certain diagnostic value for MP-infected children,their combined application further enhances diagnostic sensitivity and warrants promotion.
