摘要
对比经皮脊柱内镜下腰椎间盘摘除术(percutaneous endoscopic lumbar discectomy,PELD)与传统椎板间开窗髓核摘除术治疗腰椎间盘突出症(lumbar intervertebral disc herniation,LDH)的远期影响及疗效。回顾性分析2017年7月1日—2021年7月1日于深圳市龙华区人民医院分别采用PELD(观察组,n=20)和传统椎板间开窗髓核摘除术(对照组,n=20)进行治疗的单节段LDH患者的临床资料,比较两组患者的相关指标,并进行长期随访,对两组患者的中远期疗效进行评估。结果表明,观察组手术时间为(76.71±8.12)min,显著短于对照组的(85.42±8.23)min(P<0.05);量表评分和影像学表现方面,末次随访时,观察组的JOA评分为(25.72±3.90)分,LL为(54.82±6.23)°,均高于对照组的(23.02±4.25)分、(50.30±7.24)°,观察组SD值为(1.40±0.42)mm,低于对照组的(2.13±1.08)mm(P<0.05)。研究发现,相较于传统椎板间开窗髓核摘除术,PELD造成的手术损伤更小,更利于患者腰椎功能康复,对患者中远期预后有潜在优势。
This study compares the long-term effects and efficacy between percutaneous endoscopic lumbar discectomy(PELD)and traditional interlaminar fenestration nucleotomy in the treatment of lumbar intervertebral disc herniation(LDH).A retrospective analysis was performed on clinical data of 40 single-segment LDH patients treated with PELD(observation group,n=20)or traditional interlaminar fenestration nucleotomy(control group,n=20)at Longhua District People’s Hospital of Shenzhen from July 1,2017,to July 1,2021.Relevant indicators and long-term follow-up data were compared to evaluate the medium-and longterm efficacy.Results showed that the operation time in the observation group was(76.71±8.12)min,signifi cantly shorter than(85.42±8.23)min in the control group(P<0.05).In terms of scale scores and imaging fi ndings,the JOA score(25.72±3.90 vs.23.02±4.25)and lumbar lordosis(LL)angle(54.82±6.23°vs.50.30±7.24°)in the observation group were higher than those in the control group at the last follow-up,while the segmental displacement(SD)(1.40±0.42 mm vs.2.13±1.08 mm)was lower(P<0.05 for all).The study indicates that compared with traditional interlaminar fenestration nucleotomy,PELD causes less surgical trauma,promotes better lumbar function recovery,and demonstrates potential advantages in medium-and long-term prognosis.
