摘要
探讨并总结夏永良主任临床治疗外感后咳嗽的经验。夏师认为外感后咳嗽病机主要是气机逆乱。在临证时重在辨表里虚实,定位脏腑于肺、肾以及脾胃。其虚者多见阴虚、气虚、血虚以及肺、肾、脾胃脏腑的虚衰,实者多见痰饮和郁热,虚实夹杂者亦不少见;治疗以调和阴阳、补虚泻实为原则。遣方用药以合方为主,并且加入调畅气机的方剂或药物,配伍化裁,疗效确切。文章列举4则医案以供各位同道学习和借鉴。
This paper explored and summarized Director Xia Yongliang’s experience in the treatment of post exogenous infection cough.Through following up with the teacher and carefully studying the medical records of Professor Xia,four cases diagnosed as cough and conforming to the definition of“post exogenous infection cough”were selected,and the treatment experience of Professor Xia was discussed from the perspectives of pathogenesis,symptoms,and modified drugs.Professor Xia believed that the main pathogenesis of post exogenous infection cough is the disorder of qi.In clinical practice,the emphasis of syndrome differentiation is on distinguishing between superficial and internal deficiency and excess,and locating the organs in the lungs,kidneys,and spleen and stomach.In cases of deficiency,yin deficiency,qi deficiency,blood deficiency,and deficiency of the lungs,kidneys,spleen,and stomach organs are more common.In cases of excess,phlegm and heat stagnation are more common,but a combination of deficiency and excess is not uncommon.The principle of treatment is to harmonize yin and yang,supplement deficiency and reduce excess.In terms of prescription and medication,the main approach is to use a combination formula,and add formulas or drugs that regulate the flow of qi,which are compatible and tailored to achieve precise therapeutic effects.Professor Xia first distinguishes the exterior and interior or deficiency and excess in the diagnosis and treatment of post exogenous infection cough,combined with the specific etiology and pathogenesis,and flexibly uses the classic and timely prescriptions,
