摘要
目的 分析在对同侧上下颌骨缺损进行修复重建时,应用单侧游离腓骨肌皮瓣进行同期修复重建治疗的价值。方法 选取接受同侧上下颌软硬组织切除联合单侧游离腓骨肌皮瓣同期修复重建的30例患者,均于2021年2月至2024年6月在安阳市第六人民医院接受治疗;对患者术后恢复情况、口腔健康情况以及生存质量进行观察分析。结果 全部患者均顺利完成手术治疗,手术时间平均(391.58±20.14)min。30例患者中,术后共有29例患者的皮瓣存活情况比较理想,仅1例患者术后因为静脉血栓而导致皮瓣坏死;随访发现30例患者均没有发生显著并发症,供区愈合情况比较理想。相比于术前,患者术后1年的残障、社交障碍、心理障碍、生理障碍、心理不适、生理性疼痛、功能限制等方面评分均更低(P<0.05)。相比于术前,患者术后1年的牙齿、社会接触、饮食、张口度、语言、吞咽等方面评分均更低(P<0.05)。结论 对同侧上下颌骨缺损进行修复重建时,应用单侧游离腓骨肌皮瓣进行同期修复重建治疗是可行和安全的,能有效修复义齿,并对患者口腔健康状况及生存质量进行显著改善。
Objective To analyze the value of unilateral free fibular flap for simultaneous repair and reconstruction of ipsilateral upper and lower jaw defects.Methods A total of 30 patients who underwent ipsilateral resection of maxillary and mandibular soft and hard tissues combined with unilateral free fibular flap for simultaneous repair and reconstruction from February 2021 to June 2024 were selected.The postoperative recovery,oral health and quality of life of the patients were observed and analyzed.Results All patients successfully completed the operation.The average operation time was(391.58+20.14)minutes.Among all 30 patients,29 patients had satisfactory flap survival after operation,and only 1 patient had flap necrosis due to venous thrombosis after operation.Follow-up showed that all 30 patients had no significant complications,and the healing of the donor site was satisfactory.Compared with those before surgery,the scores of disability,social disorder,psychological disorder,physical disorder,psychological discomfort,physiological pain,and functional limitation were significantly lower 1 year after surgery(P<0.05).The scores of teeth,social contact,diet,mouth opening,language and swallowing 1 year after surgery were significantly lower than those before surgery(P<0.05).Conclusions It is feasible and safe to use unilateral free fibular flap for simultaneous reconstruction of ipsilateral maxillomandibular defects.It can effectively restore dentures and significantly improve the oral health status and quality of life of patients.
