摘要
目的探讨三维立体减张技术在面部整形切口中的应用对改善远期瘢痕的临床疗效。方法收集2023年8月至202年8月泰州市中医院烧伤整形科接受面部整形手术的患者80例临床资料进行回顾性分析,根据治疗方法不同分为对照组和察组,每组40例。对照组手术切口采用常规减张美容缝合;观察组手术切口采用三维立体减张技术处理,具体包括:真皮及筋膜层行减张美容缝合,表皮层行减张胶带减张,肌肉层行A型肉毒毒素注射减张。观察术后两组患者的手术切口愈合况及并发症。术后随访6个月采用患者与观察者瘢痕评估量表(POSAS)对切口瘢痕情况进行评分。结果术后对照组中有例患者出现手术切口红肿小部分裂开,而观察组有1例患者在面部表情微笑时出现双侧口角轻微不对称,另1例患者手术口粘贴减张胶带后局部皮肤出现红斑,两组患者并发症发生率比较差异无统计学意义(P>0.05);术后6个月POSAS量评分观察组评分低于对照组,两组比较差异有统计学意义(P<0.05)。结论在面部整形手术切口中应用三维立体减张技术过多维度张力调控可有效减轻面部切口远期瘢痕,提升美容效果,具有较高的临床应用价值,值得推广。
Objective To investigate the clinical effect of three-dimensional tension reduction technology in the application of facial plastic incision to improve the long-term scar.Methods The clinical data of 80 patients who underwent facial plastic surgery in the Department of Burn and Plastic Surgery at Taizhou Hospital of Traditional Chinese Medicine from August 2023 to August 2024 were retrospectively analyzed.The patients were divided into a control group and an observation group,with 40 cases in each group,according to the different treatment methods.The control group received conventional tension-reduction cosmetic sutures.The observation group underwent three-dimensional tension-reduction technology,which included tension-reduction cosmetic sutures in the dermis and fascia layers,tension-reduction tape in the epidermis layer,and type A botulinum toxin injection in the muscle layer.Wound healing and complications were observed in both groups.After 6 months of follow-up,the Patient and Observer Scar Assessment Scale(POSAS)was used to score the incision scars.Results In the control group,three patients exhibited surgical incision swelling and partial dehiscence postoperatively.In the observation group,one patient displayed slight asymmetry of the bilateral lips during smiling,and another patient experienced erythema on the local skin after tension-reduction tape was applied to the surgical incision.No significant difference was observed in the incidence of complications between the two groups(P>0.05).Six months after operation,the posas score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of three-dimensional tension reduction technology in facial plastic surgery incision can effectively reduce the long-term scar of facial incision and improve the cosmetic effect through multi-dimensional tension regulation,which has high clinical application value and is worthy of promotion.
