摘要
目的系统评价下肢深静脉血栓形成后综合征的风险预测模型。方法系统性检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、Web of Science、Embase、Cochrane Library数据库关于下肢深静脉血栓形成后综合征的风险预测模型研究,检索时限为建库至2024年6月30日。根据纳入排除标准独立筛选文献和提取数据,并应用预测模型研究的偏倚风险评估工具进行文献质量评价。结果共纳入15项研究20个预测模型,预测模型性能整体较好(受试者工作特征曲线下面积为0.600~0.963),但均存在偏倚风险,可能原因包括样本量不足、缺失值处理方法不当、缺乏外部验证。排名前6名的预测因子为身体质量指数、深静脉血栓形成病史、下肢静脉曲张史、年龄、髂静脉血栓形成、基线Villalta评分。结论目前的下肢深静脉血栓形成后综合征的风险预测模型可能存在一定的偏倚风险,预测因子存有一定共性与争议。
Objective To systematically evaluate the risk prediction models for post-thrombotic syndrome following deep vein thrombosis of lower extremities.Methods A systematic search was conducted in databases including CNKI,WanFang,VIP,CBM,PubMed,Web of Science,Embase and Cochrane Library for studies on risk prediction models for post-thrombotic syndrome following deep vein thrombosis of lower extremities,with the search period ranging from the inception of each database to 30 June 2024.Literature was independently screened and data were extracted based on inclusion and exclusion criteria.The prediction model risk of bias assessment tool was applied for quality assessment.Results A total of 15 studies were included,involving 20 prediction models.The overall performance of the prediction models was favorable,with the area under the receiver operating characteristic curve ranging from 0.600 to 0.963.However,all models had bias risks,potentially due to inadequate sample sizes,inappropriate handling of missing data and lack of external validation.The top six predictors identified were body mass index,history of deep vein thrombosis,history of lower extremity varicose veins,age,iliac vein thrombosis and baseline Villalta score.Conclusion The current risk prediction models for post-thrombotic syndrome following deep vein thrombosis of lower extremities still have some bias risks,with both commonalities and controversies existing among predictors.
