摘要
目的:探讨结直肠癌术后肠造口患者的社会疏离现状及其影响因素。方法:采用便利抽样法选取安徽省芜湖市某三级甲等医院胃肠外科住院及伤口造口门诊就诊的304例肠造口患者为研究对象。采用医学应对方式问卷(MCMQ)、一般疏离感量表(GAS)以及心理一致感量表(SOC)分别评估患者医学应对方式、社会疏离以及心理状况。通过Pearson法分析社会疏离、心理一致感及应对方式之间的相关性;采用多重线性回归分析结直肠癌术后肠造口患者社会疏离的影响因素。结果:肠造口患者的社会疏离得分为(35.05±7.40)分,医学应对方式各维度得分为面对(20.05±5.91)分、回避(17.02±3.34)分、屈服(11.18±4.35)分,心理一致感得分为(42.65±15.38)分;社会疏离与回避、屈服医学应对方式呈正相关(r=0.636,0.543,P<0.01),与面对医学应对方式呈负相关(r=-0.224,P<0.01);社会疏离感与可理解感、可控制感、意义感、心理一致感总分呈负相关(r=-0.295,-0.274,-0.237,-0.300,P<0.01)。结论:多数结直肠癌术后肠造口患者的社会疏离处于中等偏上水平;医务人员应关注回避和屈服医学应对方式及心理一致感较低的患者,加强家庭、社会和心理支持,鼓励患者采取积极的应对方式以降低社会疏离。
Objective:To explore the influence factors of social distancing in postoperative enterostomy patients with colorectal cancer and its correlation with medical coping style and psychological congruence.Methods:A total of 304 patients with enterostomy,who were hospitalized in gastrointestinal surgery and wound stoma clinic in a tertiary hospital,were selected as the research objects by convenient sampling method.The Medical Coping Modes Questionnaire(MCMQ),General Alienation Scale(GAS)and Sense of Coherence Scale(SOC)were used to evaluate the coping style,social alienation and psychological status of the patients.Results:The scores of social alienation in patients with colostomy were(35.05±7.40).The scores of medical coping styles of facing,avoiding and yielding were(20.05±5.91),(17.02±3.34)and(11.18±4.35),respectively,and the scores of psychological congruence were(42.65±15.38).Social distancing was positively correlated with avoidant and submissive medical coping styles(r=0.636,0.543,P<0.01),and negatively correlated with facing medical coping styles(r=-0.224,P<0.01).Social alienation was negatively correlated with comprehensibility,controllability,meaning and psychological agreement(r=-0.295,-0.274,-0.237,-0.300,P<0.01).Conclusion:The social distancing of most postoperative colostomy patients with colorectal cancer is at a moderate to high level.Medical staff should pay attention to patients with low levels of avoidant and submissive medical coping styles and psychological congruence,strengthen family,social and psychological support,and encourage patients to adopt positive coping styles to reduce social alienation.
