摘要
目的:分析2型糖尿病病人行内镜下黏膜剥离术发生低血糖的影响因素分析。方法:回顾分析2021年1月—2023年12月在江苏大学附属高淳医院消化内科行内镜下黏膜剥离术的140例2型糖尿病病人相关电子病例资料,其中发生低血糖病人24例,未发生低血糖116例,采用Logistic回归分析确定糖尿病病人行内镜下黏膜剥离术发生低血糖的影响因素。结果:2型糖尿病病人行内镜下黏膜剥离术低血糖的发生率为17.1%。Logistic回归分析显示,糖尿病病程≥10年、血糖水平的标准差(SDBG)≥3 mmol/L、体质指数(BMI)<18.5 kg/m^(2)、围术期禁食时间≥36 h、围术期禁食期间补液不合理是糖尿病病人行内镜下黏膜剥离术发生低血糖的影响因素。结论:2型糖尿病病人行内镜下黏膜剥离术低血糖发生率较高,临床护理人员应重点关注糖尿病病人病程、血糖水平的标准差、BMI、围术期禁食时间以及围术期禁食期间补液情况,术前进行综合评估、预警,并进行有效干预,从而降低内镜下手术治疗2型糖尿病病人低血糖的发生率。
Objective:To analyze the influencing factors for hypoglycemia in type 2 diabetes mellitus(T2DM)patients undergoing endoscopic submucosal dissection(ESD).Methods:A retrospective analysis was performed on the electronic medical records of 140 T2DM patients who underwent ESD in the department of gastroenterology at the Affiliated Gaochun Hospital of Jiangsu University between January 2021 to December 2023.Among them,24 patients experienced hypoglycemia,while 116 did not.Logistic regression analysis was used to identify the influencing factors for hypoglycemia in diabetic patients undergoing ESD.Results:The incidence of hypoglycemia in T2DM patients undergoing ESD was 17.1%.Logistic regression analysis demonstrated that diabetes duration≥10 years,standard deviation of blood glucose(SDBG)≥3 mmol/L,body mass index(BMI)<18.5 kg/m^(2),perioperative fasting time≥36 hours,and inappropriate fluid replacement during the perioperative fasting period were influencing factors for hypoglycemia in diabetic patients undergoing ESD.Conclusion:The incidence of hypoglycemia is relatively high in T2DM patients undergoing ESD.Clinical nursing staff should pay close attention to the patient's diabetes duration,SDBG,BMI,perioperative fasting time,and fluid replacement status during the perioperative fasting period.preoperative assessment,early warning,and effective interventions should be implemented to reduce the incidence of hypoglycemia in T2DM patients undergoing endoscopic surgery.
