MDT模式在提高狂犬病暴露者疫苗接种率和安全性中的应用研究

摘要 目的探讨多学科团队(MDT)模式在提高狂犬病暴露者疫苗接种率和安全性中的应用效果。方法选取2024年7月—2025年7月在本中心接受狂犬病疫苗接种的64例暴露者,将其分为MDT组和对照组,每组32例。对照组按照狂犬病暴露处置规范进行伤口处理和狂犬病疫苗接种。MDT组在此基础上实施MDT模式,由公共卫生医师、临床医师、护理人员等组成MDT,根据制定的工作流程对暴露者进行评估、制定个性化接种方案并实施随访管理。观察并比较两组的疫苗接种率、不良反应发生率等指标。结果MDT组32例暴露者均按时完成5针次疫苗接种,接种率为100%,明显高于对照组的81.3%(26/32)(χ^(2)=6.54,P=0.011);疫苗接种后,MDT组2例(6.3%)暴露者出现1级不良反应,对照组出现6例(18.8%)不同程度不良反应,其中1级5例、2级1例,MDT组不良反应发生率明显低于对照组(χ^(2)=7.11,P=0.029)。两组患者对医疗服务的总体满意度评分分别为(92.4±6.3)分和(80.6±11.5)分,MDT组明显更高(t=5.07,P<0.001)。结论MDT模式可提高狂犬病暴露者的疫苗接种率,减少不良反应,值得临床推广应用。 Objective To explore the application effect of Multidisciplinary Team(MDT)model in improving the vaccination rate and safety of rabies exposed people.Methods From July 2024 to July 2025,64 exposed persons who received rabies vaccination in our outpatient department were selected and divided into MDT group and control group,with 32 cases in each group.Wound treatment and rabies vaccination were carried out in both groups according to the standard of rabies exposure and treatment.On this basis,MDT group implements MDT mode,which is composed of public health doctors,clinicians and nursing staff.According to the established workflow,MDT group evaluates the exposed people,formulates personalized vaccination plan and implements follow-up management.The control group was vaccinated according to the routine procedure.Observe and compare the vaccination rate and the incidence of adverse reactions between the two groups.Results All the 32 exposed people in MDT group completed five doses of vaccination on time,and the vaccination rate was 100%,which was significantly higher than that of the control group(81.3%,26/32)(χ^(2)=6.54,P=0.011).After vaccination,2 cases(6.3%)in MDT group had first-class adverse reactions,and 6 cases(18.8%)in control group had different degrees of adverse reactions,including 5 cases in first-class and 1 case in second-class.The incidence of adverse reactions in MDT group was significantly lower than that in control group(χ^(2)=7.11,P=0.029).The overall satisfaction scores of patients in the two groups were(92.4±6.3)and(80.6±11.5)respectively,and the MDT group was significantly higher(t=5.07,P<0.001).Conclusion MDT model can improve the vaccination rate of rabies exposed people and reduce adverse reactions,which is worthy of clinical application.
