摘要
目的探讨术前吲哚菁绿(ICG)清除试验参数对肝癌患者术后肝功能不全的预测效果。方法随机抽取2023年1月—2025年1月在本院接受肝部分切除术的肝癌患者42例为研究对象。患者术前均接受ICG清除试验。采用Child-Pugh分级标准对其肝功能不全情况进行评估,并据此结果将其分为两组,即术后肝功能良好组(n=30,Child-Pugh分级A级)及术后肝功能不全组(n=12,Child-Pugh分级B级、C级),对比两组术前ICG 15 min内滞留率(ICG-R15)、ICG 10 min内滞留率(ICG-R10)、ICG 5 min内滞留率(ICG-R5)、ICG血浆清除率(K值),并通过ICG清除试验参数绘制受试者工作特征(ROC)曲线,判断其对肝癌患者术后肝功能不全的预测价值。结果肝功能良好组ICG-R 15、ICG-R 10及ICG-R 5低于肝功能不全组,ICG K高于肝功能不全组(P<0.05);ROC曲线显示,术前ICG清除试验参数预测肝癌患者术后肝功能不全的灵敏度为87.56%、特异度为85.23%、AUC为0.923。结论术前ICG清除试验参数对肝癌患者术后肝功能不全的预测效果显著。
Objective To explore the predictive value of preoperative indocyanine green(ICG)clearance test parameters for postoperative liver dysfunction in patients with liver cancer.Methods 42 liver cancer patients who underwent partial hepatectomy were randomly selected as the main subjects,and the study period was controlled from January 2023 to January 2025.All patients underwent ICG clearance test before surgery,and their liver function was evaluated by Child Pugh grading after surgery.They were divided into two groups:postoperative good liver function group(n=30,Child Pugh grading A)and postoperative liver dysfunction group(n=12,Child Pugh grading B and C).The ICG 15 minute retention rate(ICG R 15),ICG R10,ICG R5,ICG plasma clearance rate(K value)before surgery were compared between the two groups,and ICG clearance test was conducted.Draw a Receiver Operating Characteristic(ROC)curve with parameters to determine its predictive value for postoperative liver dysfunction in liver cancer patients.ICG-R 15,ICG-R 10 and ICG-R 5 in the group with good liver function were lower than those in the group with hepatic insufficiency,and ICG K was higher than that in the group with hepatic insufficiency(P<0.05).ROC curve showed that the sensitivity,specificity and AUC of preoperative ICG clearance test parameters to predict postoperative hepatic insufficiency in patients with liver cancer were 87.56%,85.23%and 0.923.Conclusion The preoperative ICG clearance test parameters have significant value in predicting postoperative liver dysfunction in liver cancer patients.
