摘要
目的探讨外伤性硬膜下血肿(tSDH)患者心理问题的影响因素。方法选择2022年1月至2023年12月在新乡医学院第二附属医院就诊及鉴定的126例tSDH患者为研究对象,根据90项症状自评量表测试结果是否异常将患者分为心理正常组(n=60)和伴心理问题组(n=66)。收集患者一般资料及影像学资料,应用单因素分析及logistic回归分析tSDH患者心理问题的影响因素。结果单因素分析结果显示,2组患者的性别、文化程度、中线移位显著、2个以上血肿、额叶血肿、颞叶血肿、顶叶血肿、蛛网膜下腔出血合并血肿、脑挫裂伤合并血肿比较差异有统计学意义(P<0.05);2组患者年龄比较差异无统计学意义(P>0.05)。Logistic回归分析结果显示,女性、文化程度高、2个以上血肿、额叶血肿、颞叶血肿、中线移位显著、蛛网膜下腔出血合并血肿、脑挫裂伤合并血肿是tSDH患者心理问题的影响因素(P<0.05)。结论tSDH患者出现心理问题的发生率较高,女性、文化程度高、2个以上血肿、额叶血肿、颞叶血肿、中线移位显著、蛛网膜下腔出血合并血肿、脑挫裂伤合并血肿是tSDH患者心理问题的影响因素。
Objective To explore the influencing factors of psychological problems in patients with traumatic subdural hematoma(tSDH).Method A total of 126 tSDH patients who were treated and identified at the Second Affiliated Hospital of Xinxiang Medical University from January 2022 to December 2023 were selected as research subjects.According to the results of the 90-item Symptom Checklist-90(SCL-90)test,they were divided into a psychological normal group(n=60)and a psychological problem group(n=66).The general information and imaging data of patients were collected,and univariate analysis and logistic regression analysis were used to explore the influencing factors of psychological problems in tSDH patients.Results The results of the univariate analysis showed that there were significant differences in gender,education level,significant midline shift,presence of two or more hematomas,frontal lobe hematoma,temporal lobe hematoma,parietal lobe hematoma,subarachnoid hemorrhage with hematoma,and cerebral contusion with hematoma of patients between the two groups(P<0.05);there was no statistically significant difference in age of patients between the two groups(P>0.05).The results of logistic regression analysis showed that female,high level of education,presence of two or more hematomas,frontal lobe hematoma,temporal lobe hematoma,significant midline shift,subarachnoid hemorrhage combined with hematoma,and cerebral contusion combined with hematoma were the influencing factors of psychological problems in tSDH patients(P<0.05).Conclusion The incidence of psychological problems in patients with tSDH is relatively high,and female,high level of education,presence of two or more hematomas,frontal lobe hematoma,temporal lobe hematoma,significant midline shift,subarachnoid hemorrhage combined with hematoma,and cerebral contusion and laceration combined with hematoma are influencing factors for psychological problems in tSDH patients.
