摘要
目的观察中药肠复方口服及灌肠对美沙拉嗪治疗无效的溃疡性结肠炎(Ulcerative colitis,UC)的临床疗效。方法采用回顾性分析方法,选取2019年6月1日—2023年6月1日北京大学人民医院中医科收治的54例UC患者,根据接受院内制剂肠复方(Chang-Fu Regimen,CFR)治疗期间,是否接受西医治疗分为联合治疗组和单纯CFR组,其中联合治疗组36例,单纯CFR组18例。所有患者均接受中药肠复方口服及灌肠治疗,根据部分梅奥评分(Partial mayo score,PMS)、单纯临床结肠炎活动指数(Simple clinical colitis activity index,SCCAI)和治疗至少4周后的肠镜检查,在第4、12和24周进行疗效评估。结果共纳入54例患者。第4周、第12周和第24周的临床临床缓解率分别为84.4%(39/46)、89.2%(33/37)和96.7%(29/30),有效率分别为50%(23/46)、54.1%(20/37)和83.3%(25/30),PMS和SCCAI在治疗后显著降低,差异有统计学意义(P<0.01,P<0.01)。第4周,单纯CFR组的临床有效率明显高于联合用药组,差异有统计学意义(P<0.01)。34例患者复查肠镜,23.5%的患者达到镜下缓解(8/34),镜下Mayo评分显著降低,差异有统计学意义(P<0.01)。两组之间的镜下有效率及镜下缓解率未见显著差异。所有患者治疗过程中未出现不良反应。结论中药肠复方口服及灌肠可以显著改善美沙拉嗪治疗无效UC患者的临床症状,并促进内镜下黏膜愈合。
Objective To investigate the clinical efficacy of compound TCM oral liquid and enema in treating Mesalazine-unresponsive Ulcerative Colitis(UC).Methods A retrospective study was conducted in this study.Fifty-four UC patients admitted to the Department of Traditional Chinese Medicine,Peking University People's Hospital from June 1st 2019 to June 1st 2023 were enrolled.These patients were divided into a combined treatment group(36 patients)and a pure Chang-Fu Regimen(CFR)group(18 patients)based on whether Western medicine was involved.All patients accepted the compound TCM oral liquid and enema.Clinical efficacy was evaluated at weeks 4,12,and 24 according to partial Mayo score(PMS),simple clinical colitis activity index(SCCAI),and colonoscopy after at least 4 weeks of treatment.Results A total of 54 patients were included.Clinical remission rates at weeks 4,12,and 24 were 84.4%(39/46),89.2%(33/37),and 96.7%(29/30),respectively.While clinical effective rates obtained 50%(23/46),54.1%(20/37),and 83.3%(25/30).After treatment,PMS and SCCAI were significantly reduced(P<0.01).At week 4,the clinical efficiency rate in the pure CFR group was significantly higher than that in the combined treatment group(P<0.01).Thirty-four patients received a colonoscopy,and 23.5%of the patients(8/34)achieved endoscopic remission and a significant decrease in endoscopic Mayo subscore(P<0.01).There were no significant differences in endoscopic efficacy and remission rate between the two groups.Besides,no adverse events occurred during the treatment.Conclusion Compound TCM oral liquid and enema can alleviate clinical symptoms in UC patients'failed response to mesalazine while promoting mucosal healing.
