摘要
目的 分析补中益气汤联合复方丙酸氯倍他索软膏(金纽尔)治疗糜烂型口腔扁平苔藓(EOLP)对患者血清神经生长因子(NGF)及唾液中差异蛋白、白细胞介素-10(IL-10)水平的影响。方法 选取医院2021年1月—2023年9月收治的80例EOLP患者并采用随机数字表法分为对照组(40例,接受金纽尔治疗)和观察组(40例,接受补中益气汤联合金纽尔治疗),比较两组患者临床疗效、中医证候积分、疼痛情况、实验室指标[血清NGF、免疫球蛋白A(IgA)、血清白蛋白(Alb)、IL-10]及不良反应。结果 治疗后,观察组患者总有效率及各项中医证候积分均优于对照组(P<0.05);治疗1周后、1个月后观察组视觉模拟(VAS)评分为[(2.21±0.52)分、(1.55±0.32)分]低于对照组的[(3.26±0.88)分、(2.13±0.58)分](P<0.05);治疗后观察组患者NGF、Alb、IL-10水平分别为[(88.47±10.49)ng/mL、(24.12±2.11)g/L、(8.39±1.68)pg/mL]低于对照组的[(102.41±11.37)ng/mL、(30.44±3.02)g/L、(12.47±1.95)pg/mL],IgA水平为(3.13±0.48)g/L高于对照组的(2.56±0.52)g/L(P<0.05);两组不良反应发生率比较差异无统计学意义(P<0.05)。结论 补中益气汤联合金纽尔治疗EOLP疗效显著,可改善患者各项中医证候积分以及疼痛情况,可调节患者血清NGF、唾液差异蛋白以及IL-10的水平,不良反应发生率低,具有安全性。
Objective To analyze the effect of Buzhong Yiqi Decoction(补中益气汤)combined with Compound Clobetasol Propionate Ointment on serum nerve growth factor(NGF)and differential proteins in saliva and interleukin-10(IL-10)in patients with erosive oral lichen planus(EOLP).Methods Eighty patients with EOLP who were admitted to the hospital from January 2021 to September 2023 were selected and randomly divided into control group(40 patients treated with Compound Clobetasol Propionate Ointment)and observation group(40 patients treated with Buzhong Yiqi Decoction combined with Compound Clobetasol Propionate Ointment).The clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,pain degree,laboratory indicators[serum NGF,immunoglobulin A(IgA),serum albumin(Alb)and IL-10]and adverse reactions were compared between the two groups.Results After treatment,the total effective rate and the TCM syndrome scores of the observation group were better than those of the control group(P<0.05).After 1 week and 1 month of treatment,the visual analogue scale(VAS)scores of the observation group[(2.21±0.52)points and(1.55±0.32)points]were lower than those of the control group[(3.26±0.88)points and(2.13±0.58points)](P<0.05).After treatment,the levels of NGF,Alb and IL-10 in the observation group[(88.47±10.49)ng/mL,(24.12±2.11)g/L and(8.39±1.68)pg/mL]were lower than those of the control group[(102.41±11.37)ng/mL,(30.44±3.02)g/L and(12.47±1.95)pg/mL],and the level of IgA in the observation group[(3.13±0.48)g/L]was higher than that in the the control group[(2.56±0.52)g/L](P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusion Buzhong Yiqi Decoction combined with Compound Clobetasol Propionate Ointment can achieve significant therapeutic effect on EOLP.The combined treatment can improve traditional Chinese medicine syndrome scores and pain,and regulate the levels of serum NGF,differential proteins in saliva and IL-10.In addition,the incidence of adverse reactions is low,and it is safe.
