摘要
目的 观察前列通窍汤治疗良性前列腺增生(BPH)合并慢性前列腺炎(CP)的效果。方法 选取2020年5月—2021年5月该院BPH合并CP患者86例,以随机数字表法简单随机分组为观察组与对照组各43例,对照组行常规西药治疗,观察组在对照组基础上联合服用前列通窍汤治疗。观察比较治疗前后尿动力学指标、IPSS评分、炎症因子水平。结果 观察组的治疗疗效优于对照组,观察组的总有效率为95.35%明显高于对照组的79.07%(P<0.05);两组治疗后最大尿流率(17.63±2.23/14.44±2.07)、最大膀胱容量(371.63±42.77/328.53±45.68)、膀胱稳定性(175.66±35.57/160.68±31.67)明显高于治疗前,且观察组明显高于对照组(P<0.05),治疗后两组最大逼尿肌压力(26.01±7.53/37.71±9.85)明显低于治疗前,且观察组明显低于对照组(P<0.05);治疗后两组IPSS(8.83±1.35/11.29±1.74)、NIH-CPSI(8.05±1.12/10.28±1.43)、生活质量评分(QOL)(2.94±0.21/3.47±0.44)明显低于治疗前,且观察组明显低于对照组(P<0.05);治疗后两组前列腺液中肿瘤坏死因子-α(TNF-α)(42.32±8.06/51.54±8.72)、白介素6(IL-6)(10.34±1.22/12.58±1.41)、白介素8(IL-8)(5.34±1.68/7.05±1.87)水平明显低于治疗前,且观察组明显低于对照组(P<0.05)。结论 对BPH合并CP患者行前列通窍汤联合西药治疗能有效提高治疗疗效,改善下尿路症状和尿动力学,减少炎症反应,提高患者术后康复生活质量。
Objective To observe the therapeutic effect of Qianlie Tongqiao Decoction(前列通窍汤)on benign prostatic hyperplasia(BPH)combined with chronic prostatitis(CP).Methods From May 2020 to May 2021,86 patients with BPH complicated with CP in the hospital were randomly divided into observation group and control group with 43 cases in each.The control group was treated with conventional western medicine.The control group was treated with Qianlie Tongqiao Decoction in combination.The urodynamic indexes,international prostate symptom score(IPSS)scores,National Institutes of Health-Chronic Prostatitis Symptom Inde(NIH-CPSI)scores,and the levels of inflammatory factors before and after treatment were observed and compared.Results The treatment efficacy in the observation group was better than that of the control group.The total response rate of the observation group was 95.35%,significantly higher than that(79.07%)of the control group(P<0.05).Maximum urinary flow rate[(17.63±2.23)mL/s,(14.44±2.07)mL/s],maximum cystometric capacity[(371.63±42.77)mL,(328.53±45.68)mL],bladder stability[(175.66±35.57)mL,(160.68±31.67)mL]were significantly higher than those before treatment,and the results of the observation group were significantly higher than those of the control group(P<0.05).The maximum detrusor pressures[(26.01±7.53)cm H 2O,(37.71±9.85)cm H 2O]were significantly lower after treatment than those before treatment,and the pressure of the observation group was significantly lower than that of the control group(P<0.05).IPSS scores[(8.83±1.35)points,(11.29±1.74)points],NIH-CPSI[(8.05±1.12)points,(10.28±1.43)points],quality of life score(QOL)[(2.94±0.21)points,(3.47±0.44)points]were significantly lower than those before treatment,and the scores of the observation group were significantly lower than those of the control group(P<0.05).The levels of tumor necrosis factorα(TNF-α)[(42.32±8.06)ng/mL,(51.54±8.72)ng/mL],interleukin 6(IL-6)[(10.34±1.22)ng/mL,(12.58±1.41)ng/mL],and interleukin 8(IL-8)[(5.34±1.68)ng/mL,(7.05±1.87)ng/mL]were significantly lower than those before treatment,and the levels of the observation group were significantly lower than those of the control group(P<0.05).Conclusion Treating BPH patients with CP and Qianlie Tongqiao Decoction combined with western medicine can effectively improve the curative effect,lower the urinary tract symptoms and urodynamics,reduce the inflammatory reaction and improve the quality of life after operation.
