摘要
目的探讨滋阴益气汤联合吗替麦考酚酯对气阴两虚型IgA肾病患儿中医证候积分及T淋巴细胞的影响。方法将80例IgA肾病患儿随机分为西医组和联合组,每组40例。西医组给予吗替麦考酚酯治疗,联合组在西医组治疗的基础上加用滋阴益气汤,比较2组的疗效及相关指标的变化情况。结果联合组的总有效率为92.50%,较西医组的75.00%明显提高(P<0.05)。治疗后,联合组的Lee分级改善率优于西医组(P<0.05),患儿的中医症候总积分、血肌酐、血尿素氮、24 h尿蛋白定量及尿沉淀红细胞数量均低于西医组(P<0.05)。治疗后,联合组的血清白细胞介素-6(interleukin-6,IL-6)、干扰素-γ(interferon-γ,IFN-γ)、CD8^(+)水平均低于西医组(P<0.05),CD4^(+)、CD4^(+)/CD8^(+)水平均高于西医组(P<0.05)。治疗期间,联合组的皮疹、腹泻等不良反应总发生率为5.00%,与西医组的15.00%比较差异无统计学意义(P>0.05)。结论对于气阴两虚型IgA肾病患儿,在应用吗替麦考酚酯的基础上,采用滋阴益气汤进行联合治疗能够提高疗效,降低中医证候积分,改善肾功能及T淋巴细胞水平。
Objective To investigate the therapeutic effects of Ziyin Yiqi Decoction combined with mycophenolate mofetil ester on traditional Chinese medicine(TCM)syndrome scores and T lymphocytes in children with IgA nephropathy of the Qi and Yin deficiency type.Methods A total of 80 children with IgA nephropathy were randomly divided into western medicine group and combined group,40 cases in each group.The Western medicine group received mycophenolate mofetil ester treatment,while the combined group received Ziyin Yiqi Decoction in addition to the Western medicine regimen.The therapeutic outcomes and changes in relevant indicators were compared between the 2 groups.Results The combined group achieved a total effective rate of 92.50%,significantly higher than the Western medicine group’s 75.00%(P<0.05).Post-treatment,the combined group exhibited a better Lee grade improvement rate compared with the Western medicine group(P<0.05),and the TCM syndrome scores,serum creatinine,blood urea nitrogen,24-hour urinary protein,and the number of urine precipitated red blood cells of the children were lower than those in the Western medicine group(P<0.05).Post-treatment,the serum levels of interleukin-6(IL-6),interferon-γ(IFN-γ),and CD8^(+)were lower in the combined group,while the CD4^(+)and the CD4^(+)/CD8^(+)ratio were higher than the Western medicine group(P<0.05).During treatment,the incidence of adverse reactions such as rash and diarrhea was 5.00%in the combined group,not significantly different from the 15.00%observed in the Western medicine group(P>0.05).Conclusion For children with IgA nephropathy of the Qi and Yin deficiency type,the combination treatment with Ziyin Yiqi Decoction on the basis of mycophenolate mofetil ester enhances therapeutic efficacy,reduces TCM syndrome scores,and improves renal function and T lymphocyte levels.
