摘要
精索静脉和卵巢静脉分别是男性和女性生殖系统血液回流的重要通道,二者的解剖位置和结构相似;其曲张反流是造成男性不育和女性慢性盆腔疼痛的重要病因,而传统外科手术存在创伤较大、并发症发生率较高等不足。近年来,泡沫硬化剂介入栓塞治疗因其微创性、安全性及经济性等优势,已逐渐成为精索静脉/卵巢静脉曲张的重要治疗手段,但目前尚无相关临床治疗规范。《中华血管外科杂志(中英文)》编辑委员会组织国内部分权威专家多次讨论和投票,系统回顾了国内外循证医学证据,明确了泡沫硬化剂在精索静脉/卵巢静脉曲张治疗的适应证、禁忌证、操作规范及并发症防治策略。本共识推荐对于符合条件(如精索静脉曲张Ⅱ~Ⅲ度、卵巢静脉反流伴疼痛)的患者优先选择该疗法,并强调术前评估及规范操作的重要性,以降低不良事件发生风险,为临床医师提供了标准化指导,以提升治疗效果和安全性。
The varicocele and pelvic varicocele are important channels for blood return in the male and female reproductive systems,respectively.They have similar positions and structures in the human body.Varicose reflux of these veins is an important cause of infertility in male as chronic pelvic pain in female.There are several disadvantages and complications on traditional surgical operations such as large incision.In recent years,interventional transcatheter foam-hardening sclerotherapy for the varicocele and pelvic varicocele has gradually become an important treatment method due to its minimally invasiveness,safety and economic concerning.However,there are currently no relevant norms on this field.The Editorial Committee Member of the Chinese Journal of Vascular Surgery systematically reviewed domestic and foreign evidence-based medical evidence,discussed and voted multiple times and clarify the indications,contraindications,operation norms and complication prevention strategies of interventional transcatheter foam-hardening sclerotherapy for the varicocele and pelvic varicocele.The consensus recommends that this therapy should be preferentially chosen on patients who meet the certain conditions(such as gradeⅡ-Ⅲvaricocele,ovarian vein reflux with pain),and emphasizes the importance of peri-operative assessment and standardized operation to reduce the risk of adverse events.It provides standardized guidance for clinical physicians to improve treatment efficacy and patient safety.
