摘要
目的:探讨长效促性腺激素释放激素激动剂(Gonadotropin-releasing hormone agonist,GnRH-a)预处理联合腹腔镜下卵巢囊肿剥除术对多囊卵巢综合征(Polycystic ovarian syndrome,PCOS)患者性激素及排卵功能的影响。方法:选取2023年1月-2024年1月本院就诊的103例PCOS患者。依据治疗方式将患者分为对照组(腹腔镜下卵巢囊肿剥除术治疗,n=51)和观察组(GnRH-a预处理联合腹腔镜下卵巢囊肿剥除术治疗,n=52)。比较两组性激素水平、术后6 m的排卵功能、排卵率、妊娠率及再次妊娠间隔时间。结果:治疗后,两组LH、E_(2)、T水平均比治疗前降低,且观察组更低(P<0.05)。治疗后,两组FSH水平均升高,且观察组更高(P<0.05)。治疗后,两组卵泡个数、最大卵泡直径、子宫内膜厚度均提高,且观察组更高(P<0.05)。治疗后,观察组再次妊娠间隔时间短于对照组,排卵率、妊娠率均高于对照组(P<0.05)。结论:对PCOS患者给予GnRH-a预处理联合腹腔镜下卵巢囊肿剥除术治疗,能改善患者性激素水平和排卵功能,提升其排卵率和妊娠率,促进再次妊娠。
Objective:To investigate the effects of gonadotropin-releasing hormone agonist(GnRH-a)preconditioning combined with laparoscopic ovarian cyst removal on Polycystic ovarian syndrome(PCOS).Effects of sex hormones and ovulation function in patients with PCOS.Methods:A total of 103 patients with PCOS who were treated in our hospital from January 2023 to January 2024 were retrospectively collected.According to the treatment methods,the patients were divided into control group(laparoscopic ovarian cystectomy,n=51)and observation group(GnRH-a pretreatment combined with laparoscopic ovarian cystectomy,n=52).The sex hormone levels,ovulation function,ovulation rate,pregnancy rate and re-pregnancy interval at 6 months after operation were compared between the two groups.Results:After treatment,the levels of LH,E_(2) and T in the two groups were lower than those before treatment,and those in the observation group were lower(P<0.05).After treatment,the levels of FSH in both groups increased,and those in the observation group were higher(P<0.05).After treatment,the number of follicles,maximum follicle diameter and endometrial thickness in the two groups increased,and those in the observation group were higher(P<0.05).After treatment,the interval time of re-pregnancy in the observation group was shorter than that in the control group,and the ovulation rate and pregnancy rate were higher than those in the control group(P<0.05).Conclusion:GnRH-a pretreatment combined with laparoscopic ovarian cystectomy for PCOS patients can improve the sex hormone level and ovulation function of patients,improve their ovulation rate and pregnancy rate,and promote re-pregnancy.
