摘要
目的分析比较内镜下黏膜切除术(EMR)、圈套器冷切除术(CSP)对结肠息肉患者的影响。方法86例结肠息肉患者,均于2021年6月-2023年6月被本院收治,随机分为两组,各43例。对照组行EMR,观察组行CSP,术后随访1个月。对比两组手术相关指标、疼痛程度、疼痛有关因子水平、并发症。结果观察组息肉切除时间为(1.87±0.35)min、手术时间为(40.25±4.36)min,短于对照组的(3.41±0.59)min、(47.58±6.37)min,并发症发生率为4.65%(2/43),低于对照组的18.60%(8/43);术后1d、3d的视觉模拟疼痛评分法评分分别为(2.57±0.31)、(1.36±0.15),均低于对照组的(4.05±0.48)、(2.14±0.29);且术后的神经生长因子为(215.24±18.46)pg/mL、5-羟色胺为(201.56±17.35)ng/L、前列腺素E2为(250.69±25.30)ng/L,低于对照组的(241.62±20.53)pg/mL、(219.73±19.46)ng/L、(271.43±28.52)ng/L,有统计学差异(P<0.05)。结论CSP创伤更小,术式时间更短,引起的疼痛更轻,有利于结肠息肉患者术后快速恢复,且并发症少,临床可推广应用。
Objective To analyze and compare the effects of endoscopic mucosal resection(EMR)and cold resection with loopers(CSP)on patients with colon polyps.Methods 86 patients with colon polyps,all admitted to our hospital from June 2021 to June 2023,were randomly divided into two groups of 43 cases each.EMR was performed in the control group,and CSP was performed in the observation group,with a 1-month postoperative follow-up.Surgery-related indexes,pain level,pain-related factor level,and complications were compared between the two groups.Results The observation group had a polyp resection time of(1.87±0.35)minutes and a surgical time of(40.25±4.36)minutes,which was shorter than the control group's(3.41±0.59)minutes and(47.58±6.37)minutes.The incidence of complications was 4.65%(2/43),which was lower than the control group's 18.60%(8/43);the scores of visual analog pain rating method were(2.57±0.31)and(1.36±0.15)in the postoperative period of 1d and 3d,respectively,which were lower than those of(4.05±0.48)and(2.14±0.29)in the control group;and the nerve growth factor in the postoperative period was(215.24±18.46)pg/mL,5-hydroxytryptamine was(201.56±17.35)ng/L,and prostaglandin E2 was(250.69±25.30)ng/L,which were lower than those of the control group of(241.62±20.53)pg/mL,(219.73±19.46)ng/L,and(271.43±28.52)ng/L,with a statistical difference(P<0.05).Conclusion CSP is less traumatic,shorter in operative time and causes less pain,which is conducive to the rapid postoperative recovery of patients with colon polyps and has fewer complications,and it can be popularized and applied in the clinic.
