摘要
目的:本研究探讨虚拟—真实内镜交替教学法在上消化道内镜培训中的应用价值。方法:将60名学员分为3组,A组(真实内镜教学法)采用床旁观摩2周联合真实内镜2周;B组(虚拟—真实内镜序贯教学法)采用虚拟内镜训练2周,然后真实内镜操作2周,C组(虚拟—真实内镜交替教学法)采用虚拟内镜1周与真实内镜1周交替训练(共4周)。训练结束后进行真实内镜考核,比较3组学员的完成率、操作成绩、操作时间、诊断正确率、患者舒适度及内镜操作各个环节间的差异。结果:(1)与A组相比,B组及C组的完成率更高、操作成绩更优、操作时间更短、患者舒适度评分更高,且C组的操作成绩及操作时间优于B组,但3组间诊断正确率无统计学差异。(2)在操作环节中,B组及C组学员在食管插入、幽门通过、十二指肠降段进入的成功率均显著高于A组,同时B组和C组黏膜损伤率较A组降低,但3组观察遗漏率上无明显差异。(3)问卷调查显示,采用虚拟内镜的B组及C组学员满意度及学习兴趣更高,且C组总体评价优于B组。结论:虚拟内镜联合教学可有效提升上消化道内镜初学者的培训效果,其中虚拟—真实交替教学法较序贯教学法更具优势。
Objective:To evaluate the effectiveness of an alternating virtual-real endoscopy teaching method in training for upper gastrointestinal(GI)endoscopy.Methods:Sixty trainees were randomly divided into three groups:Group A(Real-Endoscopy Group):2 weeks of bedside observation+2 weeks of hands-on real endoscopy;Group B(Sequential Virtual-Real Group):2 weeks of virtual endoscopy followed by 2 weeks of real endoscopy;Group C(Alternating Virtual-Real Group):Alternating 1 week of virtual and 1 week of real endoscopy for a total of 4 weeks.At the end of the training,all participants were assessed using real endoscopy.Metrics evaluated included procedure completion rate,performance score,operation time,diagnostic accuracy,patient comfort,and success rates in specific endoscopic tasks.Results:(1)Compared to Group A,Groups B and C showed significantly higher completion rates,better performance scores,shorter operation times,and higher patient comfort ratings.Group C outperformed Group B in performance and efficiency.No significant differences were found in diagnostic accuracy among the three groups.(2)For specific technical steps—esophageal insertion,pyloric passage,and duodenal entry—Groups B and C had significantly higher success rates than Group A.Mucosal injury rates were also lower in B and C,while missed lesion rates showed no significant difference.(3)Survey results indicated higher satisfaction and learning interest in Groups B and C,with Group C receiving the highest overall evaluation.Conclusion:Integrating virtual endoscopy into training significantly improves learning outcomes for beginners in upper GI endoscopy.The alternating virtual-real method offers greater advantages over the sequential approach.
